目的:观察自拟半夏泻心汤加味辅助吸入抗胆碱气雾剂治疗胃食管反流相关性支气管哮喘的疗效。方法:选取2013年5月—2015年5月三亚市中医院收治的经确诊为胃食管反流相关性支气管哮喘患者82例,随机分为观察组和对照组,各41例。对照组患者给予抗胆碱气雾剂常规对症治疗,观察组患者在对照组患者治疗的基础上接受自拟半夏泻心汤加味治疗。比较治疗前后两组患者胃食管反流症状评分、支气管哮喘控制评分、肺功能、以及中医证候疗效。结果:治疗后,观察组患者胃食管反流症状评分与治疗前比较显著下降(P<0.01);观察组患者胃食管反流症状评分显著低于同期对照组(P<0.01);两组患者支气管哮喘控制评分均较治疗前改善(P<0.01),观察组患者较同期对照组改善更加显著(P<0.01);两组患者的第1秒用力呼气容积占预计值百分比(FEV1%)和最大呼气流速占预计值百分比(PEV%)均较治疗前显著上升(P<0.01),观察组患者较同期对照组上升更加显著(P<0.01)。治疗8周后,观察组中医证候疗效明显优于对照组(P<0.05)。结论:半夏泻心汤加味辅助吸入抗胆碱气雾剂治疗胃食管反流相关性支气管哮喘患者的临床疗效显著,能有效改善患者胃食管反流症状,有效控制哮喘,改善患者肺功能。
Objective: To observe the curative effect of self-made Banxiaxiexin Decoction plus auxiliary inhaled anticholinergic aerosol on gastroesophageal reflux-related bronchial asthma. Methods: From May 2013 to May 2015, 82 patients with gastroesophageal reflux-related bronchial asthma admitted to Sanya Hospital of Traditional Chinese Medicine were randomly divided into observation group and control group, 41 cases in each group. Control group patients given anticholinergic aerosol conventional symptomatic treatment, observation group patients in the control group based on the treatment of patients receiving self-modified Banxiaxiexin Decoction treatment. Gastroesophageal reflux symptom scores, bronchial asthma control scores, lung function, and TCM syndromes were compared between the two groups before and after treatment. Results: After treatment, the score of gastroesophageal reflux symptom in the observation group was significantly lower than that before treatment (P <0.01); the score of gastroesophageal reflux symptom in the observation group was significantly lower than that in the control group (P <0.01) The bronchial asthma control score was improved compared with that before treatment (P <0.01), and the observation group had a more significant improvement than the control group in the same period (P <0.01). The forced expiratory volume per second in the two groups accounted for the predicted value (FEV1% (P <0.01). The observation group had a more significant increase (P <0.01) than the control group at the same period. After 8 weeks of treatment, the curative effect of TCM syndrome in observation group was obviously better than that in control group (P <0.05). Conclusion: Banxiaxiexin Decoction assisted inhalation of anticholinergic aerosol treatment of gastroesophageal reflux-related bronchial asthma patients with significant clinical efficacy, can effectively improve the symptoms of patients with gastroesophageal reflux, effective control of asthma and improve lung function.
