目的探讨老年人骨质疏松症与多种慢性疾病的相关性。方法选取2013年4月~2016年9月于解放军第一八一医院老年科住院患者373例,其中,60岁以上老年患者323例,在入院24 h内调查患者年龄、性别、身高、体重指数(BMI),根据既往病史及入院后检查了解其是否合并2型糖尿病、高血压、冠心病、慢性阻塞性肺疾病(COPD)、脑卒中、慢性肾脏疾病、白内障、脊椎退变、泌尿系结石、胆道结石等常见慢性疾病,同时测量L_2~L_4以及双侧股骨颈、粗隆及Ward’s三角区骨密度(BMD),取T-score低值,分别对全部患者及60岁以上老年患者进行偏相关分析。结果偏相关分析结果显示,全部患者中BMD与性别、年龄、BMI、2型糖尿病、脑卒中、白内障、胸椎退行性病变、膝关节退行性病变、胆道结石相关(P<0.05),60岁以上老年患者BMD与性别、BMI、2型糖尿病、高血压、冠心病、脑卒中、膝关节退行性病变、胆道结石有关(P<0.05)。结论 BMD与性别、BMI、2型糖尿病、脑卒中、膝关节退行性病变及胆道结石等因素相关,而老年患者BMD同时与高血压等相关,提示治疗和预防上述慢性疾病可能有助于骨质疏松的防治。
Objective To investigate the relationship between osteoporosis and various chronic diseases in the elderly. Methods From April 2013 to September 2016, 373 inpatients were enrolled in the geriatrics department of the 181st Hospital of PLA, including 323 elderly patients over 60 years old. The patients’ age, sex, height, body mass index (BMI), based on past medical history and post-admission examinations to determine whether they have type 2 diabetes, hypertension, coronary heart disease, chronic obstructive pulmonary disease (COPD), stroke, chronic kidney disease, cataract, spine degeneration, urinary calculi , Bile duct stones and other common chronic diseases, while measuring L_2 ~ L_4 and bilateral femoral neck, trochanter and Ward’s triangle bone mineral density (BMD), take low T-score, were all patients and elderly patients over the age of 60 partial related analysis. Results Partial correlation analysis showed that BMD in all patients was related to gender, age, BMI, type 2 diabetes mellitus, stroke, cataract, thoracic degenerative disease, degenerative knee joint and biliary calculi (P <0.05) BMD in elderly patients was related to gender, BMI, type 2 diabetes mellitus, hypertension, coronary heart disease, stroke, degenerative knee joint and biliary calculi (P <0.05). Conclusion BMD is related to factors such as gender, BMI, type 2 diabetes mellitus, stroke, knee degenerative disease and biliary calculi. In elderly patients, BMD is associated with hypertension and so on, suggesting that the treatment and prevention of these chronic diseases may contribute to the bone quality Loose control.
