目的:评价髋膝关节置换术患者低体温预防的最佳措施。方法:计算机检索Cochrane Library、PubMed、CINAHL、EMbase、Web of Science数据库、中国知网、万方、重庆维普、中国生物医学文献数据库,检索有关髋膝关节置换术低体温预防措施的随机对照试验,并追溯纳入文献的参考文献,检索时间为数据库建库至2019年12月;2位研究者根据纳入排出标准独立进行文献筛选、质量评价、资料提取,并使用Revman5.3软件对纳入的相关文献进行Meta分析。结果:纳入11篇文献,共608例患者。Meta分析结果显示:主要结局指标核心温度方面,充气升温装置优于电热毯、电热垫、循环水垫床等非充气升温措施[标准化均数差(SMD)为0.59,95%n CI0.27~0.92,n P=0.0004];次要结局指标术后寒战方面充气升温系统优于电热毯、电热垫、循环水垫床等非充气升温措施(n OR值为0.28,95%n CI0.14~0.58,n P=0.0006);热舒适度方面,充气升温系统效果并不显著(SMD为0.21,95%n CI-0.56~0.98,n P=0.59)。n 结论:充气升温系统与非充气升温措施比较,能够有效维持髋膝关节置换术患者的核心温度,降低术后寒战的发生,但在改善患者热舒适度方面并不明显,但由于研究数量及其统计学异质性,还需进行大样本高质量的研究以验证本研究结果。“,”Objective:To evaluate the best preventive measures for hypothermia in patients undergoing hip and knee replacement.Methods:Cochrane Library, PubMed, CINAHL, Embase, Web of Science, CNKI, Wanfang, VIP and Chinese biomedical literature database were searched by computer. Randomized controlled trials on hypothermia prevention measures in hip and knee arthroplasty were searched, and back into the literature references, the retrieval time is from database establishment to December 2019; the two researchers independently conducted literature screening, quality evaluation, and data extraction according to the inclusion exclusion criteria, and used Revman5.3 software for meta-analysis of the included literature.Results:A total of 608 patients were included in 11 literatures. The results of Meta-analysis showed that: in terms of core temperature, the main outcome index, the Forced-air Warming System was superior to the non-Forced-air Warming measure such as electric blanket, electric heating pad and circulating water mattress (n SMD=0.59, 95% n CI 0.27-0.92, n P=0.0004); in terms of secondary outcome indexes, the Forced-air Warming System was better than the non-Forced-air Warming measure such as electric blanket, electric heating pad and circulating water mattress (n OR=0.28, 95% n CI 0.14-0.58, n P=0.0006); in terms of thermal comfort, the Forced-air Warming System had no significant effect (n SMD = 0.21, 95%n CI-0.56-0.98, n P=0.59).n Conclusion:Compared with the Forced-air Warming System, the non-Forced-air Warming measure can effectively maintain the core temperature of patients undergoing hip and knee arthroplasty, reduce the occurrence of postoperative chills, but in the aspect of improve thermal comfort of patients is not obvious. However, due to the number of studies and their statistical heterogeneity, large sample and high-quality studies are needed to verify the results of this study.
