【目的】探讨血浆颗粒蛋白前体(PGRN)与糖尿病视网膜病变(diabetic retinopathy,DR)的相关性。【方法】选取糖尿病(DM)患者200例,所有患者均进行眼底镜检查,根据眼底镜检查结果分组:无 DR 患者79例(DM 组),单纯期 DR 患者67例(NDR 组),增殖期 DR 患者54例(PDR 组),比较各组患者血浆总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)和空腹血糖(FPG)、糖化血红蛋白(HbA1c)水平、空腹胰岛素(Fins),计算 HOMA 胰岛素抵抗指数(HOMA-IR)。检测并比较血浆颗粒蛋白前体(PGRN)、血管内皮生长因子(VEGF)、干扰素-6(IL-6)、超氧化物歧化酶(SOD)活性和谷胱甘肽(GSH)水平。【结果】三组患者的性别构成、年龄、FPG 比较差异无统计学意义(P >0.05);患者的病程、体质量指数(BMI)、HbA1c、Fins、HOMA-IR 和血脂水平比较有统计学意义(P <0.05);三组患者的 PGRN、VEGF、IL-6、SOD 和GSH 水平比较均有统计学差异(P <0.05)。PGRN 水平与患者的 HOMA-IR、IL-6、VEGF、SOD 和 GSH 水平有明显相关性(P 0.05);The course of disease,body mass (BMI),Fins,HOMA-IR, HbA1c and blood lipid levels were statistically significant (P <0.05 );There were significant differences in PGRN , VEGF,IL-6 ,SOD and GSH between the three groups (P <0.05).The level of PGRN was significantly correlated with the levels of HOMA-IR,IL-6,VEGF,SOD and GSH (P <0.05).[Conclusion]The level of PGRN in patients with DR was significantly increased,and PGRN may be involved in the development of DR by regulating insulin resistance,chronic inflammation and oxidative stress.
