多巴酚丁胺(dobutamine)是一种心脏选择性肾上腺素能制剂,它能增加心肌收缩力、心搏出量及心肌血流量,而对心率及血压影响很小。临床上可用于改善心功能,但不损害缺血心肌的氧平衡及代谢。在实验性急性心肌梗塞中,显示有减小梗塞范围的作用。由于多巴胺能使局部去甲肾上腺素的释放增加,因此在急性心肌梗塞中,多巴胺可使心肌缺血损害加重。本文试通过实验性急性心肌梗塞的清醒狗,比较一下多巴酚丁胺与多巴胺对系统血液动力学、心肌血流量及梗塞范围方面的作用。
Dobutamine is a cardioselective adrenergic agent that increases myocardial contractility, stroke volume, and myocardial blood flow with little effect on heart rate and blood pressure. Clinically available for improving cardiac function, but does not damage the oxygen balance and metabolism of ischemic myocardium. In experimental acute myocardial infarction, the effect of reducing infarct size has been shown. As dopamine increases the release of norepinephrine, dopamine may worsen myocardial ischemia in acute myocardial infarction. This article tries to compare the effects of dobutamine and dopamine on systemic hemodynamics, myocardial blood flow, and infarct size in conscious awake dogs with experimental acute myocardial infarction.
