目的应用超声的实时剪切波弹性成像(SWE)技术对腮腺内实性良性包块进行弹性模量测量,探讨其在腮腺内实性良性包块鉴别诊断中的临床价值。方法选择168例腮腺内包块行外科手术或病理组织活检的患者,包括混合瘤65例,腺淋巴瘤45例,淋巴结58例,每例患者手术前或病理组织活检前均行SWE,得到腮腺内实性良性包块弹性模量的最大值及平均值,3组患者的弹性模量最大值及平均值进行组间比较。结果混合瘤组弹性模量最大值(158.68±3.36)kPa,平均(130.67±3.24)kPa;腺淋巴瘤组弹性模量最大值(98.89±3.30)kPa,平均(67.60±2.03)kPa,淋巴结组弹性模量最大值(54.99±2.36)kPa,平均(34.22±2.24)kPa,3组均值不来自同一总体,组间具有可比性。每两组之间的弹性模量最大值及平均值相比有统计学意义(P<0.001)。结论SWE技术能分辨腮腺内不同实性良性包块的性质,为临床医师明确诊断腮腺内实性包块进一步提供理论依据。
Objective To evaluate the elastic modulus of real parotid mass in the parotid gland with real-time shear wave elastography (SWE), and to explore its clinical value in the differential diagnosis of solid parotid masses. Methods A total of 168 patients with parotid gland internal tumor undergoing surgery or pathological biopsy were selected, including 65 cases of mixed tumor, 45 cases of adenolymphoma and 58 cases of lymph node. SWE was performed before operation or in pathological tissue biopsy in each patient. The maximum and average values of the elastic modulus of the solid benign mass and the maximum and the average of the elastic modulus of the three groups were compared between groups. Results The maximum elastic modulus of mixed tumor group was (158.68 ± 3.36) kPa, average (130.67 ± 3.24) kPa; the maximum value of elastic modulus was 98.89 ± 3.30 kPa (67.60 ± 2.03) kPa, lymph node group The maximum elastic modulus (54.99 ± 2.36) kPa, mean (34.22 ± 2.24) kPa, three groups mean not from the same overall, comparable between groups. The maximum and average elastic modulus between each two groups were statistically significant (P <0.001). Conclusion SWE technique can distinguish the nature of different benign masses in the parotid gland and provide a theoretical basis for the clinicians to diagnose the solid mass in parotid glands.
