目的研究根据CT血流灌注、功能性磁共振成像(f MRI)结果指导治疗和应用巴戟天低聚糖巴戟甲素治疗血管性痴呆(VD)的近期和远期效果。方法根据检测和治疗手段的差异,将患者随机分为4组:CT血流灌注组、f MRI组、巴戟甲素组、对照组。比较相关参数,分析几种疗法的治疗效果。结果出院时,血流灌注组的左额叶脑血流量(CBV)、右额叶CBV、左基底节峰值时间(TTP)、右基底节TTP等血流灌注参数分别为1.6、1.4、13.9、15.5,而对照组患者分别为2.1、2.5、18.2、18.5。出院1年后,实验组A患者的左额叶CBV、右额叶CBV、左基底节TTP、右基底节TTP的血流灌注参数分别为1.8、1.3、13.3、15.7,而对照组患者分别为2.2、2.4、18.1、18.6。出院时,实验组B患者治疗后在右顶额中回、左顶额中回、左侧小脑蚓、右侧小脑蚓几个检测区域的像素值分别是102、131、35、302;而对照组的患者分别是283、250、680、491。出院1年后,实验组B患者治疗后在右顶额中回、左顶额中回、左侧小脑蚓、右侧小脑蚓几个检测区域的像素值分别是105、135、39、310,而对照组患者分别是288、255、688、494。在出院时和出院1年后,服用巴戟甲素治疗患者的识字能力、自理能力、与人交往能力以及对于治疗的满意率明显高于对照组(均P<0.05)。结论 CT血流灌注、f MRI和巴戟天低聚糖巴戟甲素治疗VD无论从近期还是从远期效果来看均有明显效果。
Objective To study the short-term and long-term effects of treatment and application of Radix Morinda officinalis Radix on vascular dementia (VD) guided by CT perfusion and f-magnetic resonance imaging (f MRI). Methods According to the difference of detection and treatment methods, the patients were randomly divided into 4 groups: CT perfusion group, f MRI group, Bajiao A group and control group. Compare the relevant parameters, analysis of the therapeutic effect of several therapies. Results At discharge, the perfusion parameters of left frontal cerebral blood flow (CBV), right frontal lobe CBV, left basal ganglion peak time (TTP) and right basal ganglia TTP were 1.6, 1.4, 13.9, 15.5, while the control group patients were 2.1,2.5,18.2,18.5. One year after discharge, the blood perfusion parameters of left frontal lobe CBV, right frontal lobe CBV, left basal ganglia TTP, and right basal ganglia TTP in experimental group A were 1.8, 1.3, 13.3 and 15.7 respectively, while those in control group were 2.2, 2.4, 18.1, 18.6. At discharge, the experimental group B patients after treatment in the right top of the middle back, left middle frontal gyrus, left cerebellum, right cerebellar several detection area pixel values were 102,131,35,302; and control Patients in the group were 283,250,680,491 respectively. One year after discharge, the experimental group B patients after treatment in the right top of the middle back, left top of the back, left cerebellar, right cerebellar several detection area pixel values were 105,135,39,310, The control group patients were 288,255,688,494. At discharge and 1 year after discharge, the patients’ ability of literacy, self-care ability, interaction with people and satisfaction rate of treatment were significantly higher than those of the control group (all P <0.05). Conclusion CT perfusion, f MRI and Morinda officinalis Baji treatment of VD both in the short term and from the long-term results have a significant effect.
