本研究进行新型光纤CO2激光(CO2 laser wave guide,LWG)机器人外科手术(transoral robotic surgery,TORS)可行性分析,纳入年龄大于18岁的恶性头颈肿瘤患者共4例,分别患T1期舌底癌,T1及T2期声门上型喉癌,扁桃体癌,平均年龄56岁,4例均已签署知情同意书。手术器械包括马里兰分离钳,0°内镜,光纤CO2激光机器人手术臂。激光参数如下:超脉冲或连续波模式,功率7~15 W,连续曝光,平均准备时间30 min,平均手术时间94 min,每台手术使用一根光纤,平均组织凝固深度200μm(范围:100~300μm)。术中均未出现并发症,平均住院时间6 d。光纤CO2激光对于机器人外科手术是一个可靠引导工具,它可使手术时间缩短1 h有余而不带来负面后果。
In this study, a feasibility study of a new type of transoral robotic surgery (TORS) using CO2 laser wave guide (LWG) was performed in 4 patients with malignant head and neck cancer older than 18 years old, , T1 and T2 supraglottic laryngeal cancer, tonsil cancer, mean age 56 years old, 4 cases have been signed informed consent. Surgical instruments include Maryland detachment forceps, 0 ° endoscope, fiber CO2 laser robotic arm. Laser parameters were as follows: ultra-pulse or continuous wave mode, power 7-15 W, continuous exposure, average preparation time 30 min, average operation time 94 min, each using a single optical fiber, the average tissue coagulation depth 200μm (range: 300 μm). No complications occurred during the operation, the average length of stay was 6 days. Fiber CO2 laser is a reliable guidance tool for robotic surgery, which shortens the operation time by 1 h without negative consequences.
