目的探讨小儿长骨尤文氏肉瘤的超声图像特征及诊断价值。方法回顾分析了29例手术证实为尤文氏肉瘤的患儿资料,总结超声图像特征,并对照X线影像学及病理结果研究。结果 29例儿童长骨尤文氏肉瘤,肱骨11例、股骨7例、胫骨7例、桡骨及腓骨各2例。超声表现:(1)病变沿骨长轴扩展,骨皮质呈虫蚀样改变。(2)骨膜反应明显,可表现为多层状,可见Codman三角。(3)包绕病变骨可见实性低回声组织肿块,血流丰富,肿块内可取到高阻的动脉血流频谱。结论儿童长骨尤文氏肉瘤具有明显特征的超声图像表现,可为临床提供诊断及鉴别诊断信息。
Objective To investigate the ultrasonographic features and diagnostic value of pediatric long bone Ewing’s sarcoma. Methods The data of 29 children with Ewing’s sarcoma confirmed by operation were retrospectively analyzed. The characteristics of ultrasound images were summarized and compared with X-ray imaging and pathological findings. Results Eighteen cases of long bones Ewing’s sarcoma, 11 cases of humerus, 7 cases of femur, 7 cases of tibia, 2 cases of radial and fibular. Ultrasound findings: (1) lesions along the long axis of bone expansion, cortical bone samples were worm-like changes. (2) periosteal reaction was obvious, can be expressed as a multi-layered, visible Codman triangle. (3) around the lesion visible solid low hypoechoic mass, rich in blood flow, the mass can be taken within the high impedance arterial blood flow spectrum. Conclusion The long bones of Ewing’s sarcoma have obvious features of ultrasound images, which can provide diagnostic and differential diagnosis information for clinic.
