目的探讨人乳头瘤病毒(human papillomavirus,HPV)L1蛋白在宫颈病变中的表达及其与HPV16/18、年龄等相关因素的关系。方法选取2011年10月至2013年3月在郑州大学第一附属医院就诊的HPV感染者496例,检测并分析其HPV L1蛋白与宫颈病变、HPV16/18感染、年龄等的关系。结果组织病理确诊宫颈病变的患者中宫颈上皮内瘤变(cervical intraepithelial neoplasia,CIN)I组中HPV L1蛋白的阳性率高于慢性宫颈炎(chronic cervicitis,CC)组、CINⅡ/CINⅢ组、鳞状细胞癌(squamous cervical cancer,SCC)组(P<0.05)。宫颈液基细胞学检查中低度鳞状上皮内病变(low-grade squamous intraepithelial lesion,LISL)组中HPV L1蛋白的阳性率高于正常范围(negative for intraepithelial lesion,NILM)、意义不明的不典型鳞状上皮(atypical squamous cells of undetermined significance,ASC-US)、不能排除高度上皮内病变的不典型鳞状上皮细胞(atypical squamous cells-cannot exclude HIS,ASC-H)/高度鳞状上皮内病变(high-grade squamous intraepithelial lesion,HSIL)组(P<0.05)。按年龄分层,CINⅡ及其以上病变组中≤40岁组HPV L1蛋白的阳性率高于>40岁组(P<0.05)。HPV L1蛋白的阳性表达率与16/18型HPV感染呈负相关(r=-0.322)。结论 HPV L1蛋白的表达可能与宫颈病变程度相关。HPV16/18感染的患者HPV L1蛋白的阳性率低于其他HPV亚型感染,HPV L1蛋白的阳性率在CINⅡ及以上宫颈病变中与年龄有一定的关系。
Objective To investigate the expression of human papillomavirus (HPV) L1 protein in cervical lesions and its relationship with HPV16 / 18, age and other related factors. Methods 496 cases of HPV infection in the First Affiliated Hospital of Zhengzhou University from October 2011 to March 2013 were selected for detection and analysis of the relationship between HPV L1 protein and cervical lesions, HPV16 / 18 infection, age and so on. Results The positive rate of HPV L1 protein in cervical intraepithelial neoplasia (CIN) group I was significantly higher than that in chronic cervicitis (CC) group, CINⅡ / CIN Ⅲ group and squamous Squamous cervical cancer (SCC) group (P <0.05). The positive rate of HPV L1 protein in the low-grade squamous intraepithelial lesion (LISL) group in cervical liquid-based cytology was higher than that of negative for intraepithelial lesion (NILM), and the atypical atypical stage Atypical squamous cells of undetermined significance (ASC-US), atypical squamous cells-can not exclude HIS (ASC-H) / highly squamous intraepithelial lesion high-grade squamous intraepithelial lesion (HSIL) group (P <0.05). According to age stratification, the positive rate of HPV L1 protein in ≤40 years old group was higher than that in> 40 years old group (P <0.05). The positive rate of HPV L1 protein was negatively correlated with HPV type 16/18 (r = -0.322). Conclusion HPV L1 protein expression may be related to the degree of cervical lesions. The positive rate of HPV L1 protein in patients with HPV16 / 18 infection was lower than those in other HPV subtypes. The positive rate of HPV L1 protein was related to age in CINⅡ and above cervical lesions.
