目的研究对胰岛素、复方丹参、美西律治疗糖尿病合并周围神经病变的临床观察。方法在严格控制饮食的基础上,根据患者的血糖情况,分别给予胰岛素16～25U(每日总量)皮下注射至患者血糖恢复正常,同时给予复方丹参20ml静脉滴注,1次/d,美西律片200mg,3次/d口服,4周为1个疗程。结果给药后1个疗程治愈140例,占87%,好转16例,占10%,无效4例,占2%,总有效率为97%。结论糖尿病合并周围神经病变是糖尿病常见的病发症,尤其是老年人患病率更高,目前在治疗上是很不理想的,无特效治疗方法,老年性糖尿病由于胰岛素分泌不足及糖脂肪代谢紊乱,导致循环障碍及外周神经病变,因此,本文认为控制血糖在正常范围是治疗本病的关键,然后采用复方丹参注射液能改善微循环和组织缺氧,促进周围神经功能恢复。在严格控制饮食、调节胰岛素剂量、控制血糖和改善微循环上对合并周围神经病变引起的疼痛,以往多使用多种B族维生素和非甾体类抗炎药,治疗效果不佳。改用美西律治疗均有较好的镇痛作用,这可能是美西律对钠通道的阻滞作用,抑制神经纤维再生的自发活动有关。本疗法疗效可靠,无明显的毒副作用,值得临床推广,而且价格低廉,易被患者所接受。 Objective To study the effects of insulin, salvia miltiorrhiza and mexiletine on diabetic peripheral neuropathy.
