目的:研究肾损伤分子1(KIM-1)在扩张型心肌病(DCM)心力衰竭患者中肾功能的评估作用。方法:入选DCM心力衰竭患者68例以及同一年龄段的健康对照组68例,行超声心动图检查,检测血清肌酐、pro-BNP、尿中KIM-1的浓度,研究两组的pro-BNP、左心室射血分数(LVEF)、肌酐清除率(Ccr)、尿KIM-1变化情况。结果:与对照组相比,心衰组LVEF、Ccr明显降低,pro-BNP、KIM-1明显升高(P<0.05);心衰组中,随着心衰症状加重LVEF明显降低,pro-BNP、KIM-1明显升高(P<0.05),Ccr虽有降低,但差异无统计学意义(P>0.05);心衰组KIM-1与pro-BNP、LVEF、Ccr均明显相关(r=0.831、-0.728、-0.677,P<0.01)。结论:KIM-1能较早地反映DCM心力衰竭患者的肾功能损伤,对心力衰竭患者的病情评估、预后判断有较大价值。
AIM: To assess the renal function of renal injury molecule 1 (KIM-1) in patients with dilated cardiomyopathy (DCM). Methods: Sixty-eight patients with DCM heart failure and 68 healthy controls of the same age were enrolled in this study. Echocardiography was used to detect serum creatinine, pro-BNP and urinary KIM-1 concentration. The levels of pro-BNP, Left ventricular ejection fraction (LVEF), creatinine clearance (Ccr), urine KIM-1 changes. Results: Compared with the control group, LVEF and Ccr were significantly decreased and pro-BNP and KIM-1 were significantly increased in the CHF group (P <0.05). In the HF group, the LVEF and pro- BNP and KIM-1 were significantly increased (P <0.05). Although Ccr was decreased, the difference was not statistically significant (P> 0.05); KIM-1 was significantly correlated with pro-BNP, LVEF and Ccr = 0.831, -0.728, -0.677, P <0.01). Conclusion: KIM-1 can reflect the renal function damage of DCM patients with heart failure earlier and has more value in the assessment and prognosis of heart failure patients.
