目的 对 2 0 0 4上半年我院急诊中心抗生素类药品使用消耗与配伍联用进行分析 ,为临床医生合理用药和医院对抗生素使用管理提供参考。方法 使用药房计算机管理系统分析 2 0 0 4上半年度抗菌药出库情况 ,做出分类统计 ,并做出抗菌药物的口服与注射剂用药频数DDDs分析 ;同时随机抽查急诊中心药房 6个月中 5 5 2 7张处方 ,考察其中抗菌药的使用现状及主要配伍联用的合理性。结果 急诊药房抗菌药物共 8大类 4 5个品种 ,品种数不足药品总数 (36 0余种 )的 1/8,但消耗金额却占到 6 2 . 3%。DDDs统计分析使用最多的两种剂型 ,显示使用频次最高的 10种抗菌药物。结论 近年青霉素和氨苄青霉素耐药菌增长势头较大 ,所以临床更多转向广谱青霉素和二、三代及复方头孢菌素类。急诊初诊患者较多 ,少见复杂混合感染 ,单用抗菌药占 80 . 8% ,基本符合抗菌药物的使用特点。因病人发病突然 ,诊断救治的时间也相当紧迫 ,医师接诊仅据简单生化检测和诊治经验用药 ,加之抗生素类新药频现而各具药理学特点 ,部分医师选择用药欠缺合理性 ;联合用药抗菌谱求大求全 ,抗生素的使用方式和联用效力还有待改进。
Objective To analyze the consumption and compatibility of antibiotics used in emergency department of our hospital in the first half of 2004 so as to provide reference for the rational use of drugs by clinicians and the administration of antibiotics in hospitals. Methods The pharmacy computer management system was used to analyze the outpatients of antibacterials during the first half of 2004 and to make the classification statistics and to make DDDs analysis of oral and injectable drugs for antibacterials. At the same time, 5 2 7 prescriptions, to investigate the status of the use of antibacterial drugs and the compatibility of the main compatibility. Results A total of 45 kinds of emergency pharmacy antibacterial drugs were found in 8 categories and the number of varieties was less than 1/8 of the total number of drugs (36 0 species), but the consumption amount accounted for 62.3%. The two most used forms of DDDs for statistical analysis showed the 10 most frequently used antimicrobials. Conclusion In recent years, penicillin and ampicillin resistant bacteria growth momentum, so the more clinical shift to broad-spectrum penicillin and second and third generation and compound cephalosporins. Early diagnosis of emergency patients more rare mixed infection, antibacterial accounted for 80.8%, basically in line with the characteristics of the use of antibacterial drugs. Due to the patient’s sudden onset, the diagnosis and treatment of the time is quite urgent, the doctor admissions are based on simple biochemical tests and diagnosis and treatment of experience medication, combined with antibiotics often appear pharmacological characteristics of new drugs, some doctors choose to use drugs is not reasonable; combination antibacterial Spectrum seeking big demand, the use of antibiotics and combined efficacy still needs improvement.
