目的 探讨影响准分子激光屈光性角膜切削术 (laserphotorefractivekeratectomy,PRK)后眼压测量值变化的因素。方法 对 2 0 9例 (36 4只眼 )近视患者行PRK ,手术前、后采用Goldemann压平式眼压计测量患者的眼压 ,并测量角膜厚度和角膜曲率 ,进行统计学分析。术后随访时间 >1年。结果 术后 6个月和 12个月术眼的眼压测量值低于术前 ,差异有非常显著意义 (P <0 0 0 1) ,术后 12个月的眼压测量值低于术后 6个月 ,差异有非常显著意义 (P <0 0 0 1)。术后眼压测量值的降低与角膜厚度的减少和角膜曲率的降低均呈高度正相关 (P <0 0 1) ,其二元回归方程为Y(手术前、后眼压差 ,mmHg) =1 15 6 +0 0 2 2X1(手术前、后角膜厚度差 ,μm) +0 0 5 2X2 (手术前、后角膜曲率差 ,D)。结论PRK术后眼压测量值低于术前 ;角膜厚度和角膜曲率是导致手术前、后眼压测量值变化的因素。
Objective To investigate the changes of intraocular pressure (IOP) after laser photorefractive keratectomy (PRK). Methods PRK was performed in 209 patients (364 eyes) with myopia. The intraocular pressure (IOP) was measured by Goldemann tonometer before and after surgery. The corneal thickness and corneal curvature were measured and statistically analyzed. Follow-up time> 1 year. Results The intraocular pressure measured at 6 months and 12 months postoperatively was lower than that before operation (P <0.01). The intraocular pressure measured at 12 months postoperatively was lower than that after operation At 6 months, the difference was significant (P <0.01). The decrease of IOP after surgery was positively correlated with the decrease of corneal thickness and the decrease of corneal curvature (P <0.01), and the binary regression equation was Y (before and after IOP, mmHg) = 1 15 6 +0 0 2 2X1 (difference in corneal thickness before and after surgery, μm) +0 0 2X2 (difference in corneal curvature before and after surgery, D). Conclusions The intraocular pressure (IOP) measurement after PRK is lower than that before operation. Corneal thickness and corneal curvature are the factors that cause the change of intraocular pressure (IOP) before and after surgery.
