目的:探讨缺血修饰白蛋白(IMA)水平在急性冠状动脉综合征(ACS)早期诊断中的价值。方法:选择2012年3~12月收治的85例ACS患者作为研究对象,在患者胸痛3 h内、6 h分别测定血清IMA、肌红蛋白(Mb)、肌钙蛋白-I(cTnI)、B型钠酸钛(BNP)和肌酸激酶同工酶(CK-MB质量),并与50例健康体检者(对照组)进行比较。结果:3 h组血清IMA和Mb水平与对照组比较,差异有统计学意义(P<0.01);6 h组IMA、Mb、cTnI、BNP和CK-MB质量水平与对照组比较,差异有统计学意义(P<0.01)。3h组的IMA诊断敏感度最高(82.4%);6 h纽的Mb诊断敏感度最高(92.9%),IMA次之(74.1%)。结论:与其他心肌标志物相比,IMA在心肌缺血早期有更高的灵敏度,对于胸痛发病时间小于3 h的ACS更有价值,IMA与Mb、cTnI、BNP和CK-MB质量的联测,有助于ACS的早期诊断。
Objective: To investigate the value of ischemic-modified albumin (IMA) in the early diagnosis of acute coronary syndrome (ACS). METHODS: Eighty-five ACS patients admitted from March 2012 to December 2012 were enrolled in this study. Serum IMA, Mb, cTnI, B (BNP) and creatine kinase (CK-MB), and compared with 50 healthy subjects (control group). Results: The levels of IMA and Mb in 3 h group were significantly different from those in control group (P <0.01). The levels of IMA, Mb, cTnI, BNP and CK-MB in 6 h group were significantly lower than those in control group Significance (P <0.01). The diagnostic sensitivity of IMA in 3h group was the highest (82.4%). The sensitivity of Mb in 6h was highest (92.9%) and IMA was the second (74.1%). CONCLUSIONS: Compared with other cardiac markers, IMA has higher sensitivity in early stage of myocardial ischemia, more valuable in ACS with onset of chest pain less than 3 h and joint detection of IMA with Mb, cTnI, BNP and CK-MB , Contribute to the early diagnosis of ACS.
