目的:探讨上颌窦底提升不植骨同期植入种植体的临床疗效。方法:选取36例上颌后牙缺失患者按收治时间顺序分为对照组18例(可用骨高度5~8mm,共种植21颗)和观察组(可用骨高度4~5mm,共种植23颗),均进行上颌窦底提升术,观察组进行上颌窦底提升不植骨同期植入种植体进行修复,对照组上颌窦提升植骨同期植入种植体进行修复,修复6个月后,比较两组种植体的存留率,随访1年,记录两组新骨获得量及牙周探诊深度。结果:两组种植体存留率比较差异无统计学意义;观察组新骨获得量(2.92±0.49)mm、牙周探诊深度(2.87±0.35)mm均与对照组相比差异不显著(P>0.05),观察组术后3周骨密度为(0.041±0.010)、4周(0.091±0.024)、8周(0.135±0.043),8周后与前期相比,差异具有统计学意义(P<0.05)。结论:上颌窦提升植骨同期植入种植体和不植骨粉同期植入种植体取得同样的治疗效果,疗效肯定,均适用于上颌后牙缺失者,具有较高成功率。
Objective: To investigate the clinical effect of maxillary sinus floor lifting implant without implants. Methods: Totally 36 cases of maxillary posterior teeth missing were divided into control group (18 cases), control group (21 cases) and observation group (available bone height 4 ~ 5mm, total 23) All patients underwent maxillofacial ascending surgery. In the observation group, maxillary sinus floor lifting without implants implantation was performed in the same period. In the control group, maxillary sinus augmentation and bone graft were implanted with implants for repair. After 6 months of repair, Implant survival rate, follow-up of 1 year, record the amount of new bone acquisition and periodontal exploration depth. Results: There was no significant difference in survival rate between the two groups. There was no significant difference between the two groups (2.92 ± 0.49) mm and 2.87 ± 0.35 mm (P> > 0.05). The BMD of the observation group was (0.041 ± 0.010), 4 weeks (0.091 ± 0.024) and 8 weeks (0.135 ± 0.043) respectively at 3 weeks after operation, and the difference was statistically significant after 8 weeks <0.05). CONCLUSIONS: Simultaneous implantation of implants and non-grafted bone implants in maxillary sinus augmentation can achieve the same therapeutic effect with certain curative effect. All of them are suitable for the treatment of maxillary posterior teeth with high success rate.
