幽门螺杆菌(HP)与消化性溃疡(PU)疾病之间的关系日渐明确,但HP与PU之间是否有直接关系未见明确报道。本文对胃镜确诊为PU的256例(男180例、女76例),平均年龄42.6岁的患者进行溃疡处和胃窦部粘膜各取2块组织,每处2块组织分别做1分钟尿素酶试验和PCR HP的DNA测定,发现溃疡处均为阳性220例(85.9%)、阴性36例(14.1%),胃窦部粘膜均为阳性212例(82.8%)、阴性44例(17.2%)。其中各种溃疡处HP阳性率分别为:十二指肠溃疡84.4%,胃溃疡78.6%,幽门管溃疡100%,复合性溃疡100%,吻合口溃癌100%。PU患者总阳性率高,而且溃疡处阳性率高于胃寞部,提示HP可能为PU形成的直接致病因素有关,其机制可能与HP菌体和鞭毛特殊结果以及菌体的酶、细胞毒素对胃粘膜屏障的直接损害或加上胃酸的作用有关。
The relationship between Helicobacter pylori (HP) and peptic ulcer disease (PU) is increasingly clear but there is no clear indication of the direct relationship between HP and PU. In this paper, 256 cases (180 males and 76 females) diagnosed with gastroscopy were divided into two groups: ulcer and mucosa of gastric mucosa. The average age was 42.6 years. Two tissues were taken for 1 minute urease 220 cases (82.8%) were positive in mucosa of gastric antrum and 44 cases (17.2%) were negative in gastric mucosa of 220 cases (85.9%) and 36 cases . The positive rate of HP in various ulcers was 84.4% for duodenal ulcer, 78.6% for gastric ulcer, 100% for pyloric ulcer, 100% for combined ulcer and 100% for ulcer. PU patients with high positive rate, and the positive rate of gastric mucosa ulcers at higher rates, suggesting that HP may be directly related to the formation of PU-related factors, the mechanism may be associated with HP bacteria and flagella special results and bacterial enzymes, cytotoxins Direct damage to the gastric mucosal barrier or with the role of gastric acid.
