目的了解本地区O型及Rh阴性孕产妇的新生儿溶血病(HDN)的发病情况。方法对169例O型及73例Rh阴性妊娠期妇女进行“产前检查”检测;母婴156例ABO血型不合和54例Rh血型不合患儿进行新生儿溶血“3项试验”检测。结果在夫妻ABO血型不合的169例中,妻子血清中IgG抗-A或抗-B效价≥64者为85例(50.3%)。73例Rh阴性妇女中,夫妻Rh不合ABO血型相合者58例,抗-D效价32者10例(13.7%),抗-D效价16者17例(23.3%);Rh合并ABO血型不合者15例,抗-D均为阴性,其中IgG抗-A或抗-B效价≥64者为10例。母婴ABO血型不合156例,发生ABO HDN有95例,占60.8%(95/156)。母婴Rh血型不合54例,发生Rh HDN者为29例,占53.7%(29/54)。对比HDN和非HDN者的胆红素和血红蛋白值结果无明显区别。结论对夫妻血型不合的孕妇进行血型血清学检查,可提前预测HDN发生的可能性,通过产前治疗,减少HDN的发生。曾发生不明原因的流产的妇女进行妊娠前检测,可明确因夫妻血型不合造成的流产,为临床治疗提供依据。
Objective To understand the incidence of neonatal hemolytic disease (HDN) in O and Rh-negative pregnant women in this area. Methods One hundred and ninety-nine O-type and 73 Rh-negative pregnant women were tested for “prenatal care”, 156 cases of neonatal hemolytic disease, "156 cases of maternal-fetal ABO incompatibility, and 54 cases of Rh-type disability. Detection. Results Among the 169 cases of marital ABO incompatibility, 85 cases (50.3%) had an IgG anti-A or anti-B titer≥64 in the wife’s serum. There were 156 cases of ABO blood group incompatibility in maternal and infant, 95 cases occurred ABO HDN, accounting for 60.8% (95/156). 54 cases of maternal and child Rh incompatibility, Rh HDN occurred in 29 cases, accounting for 53.7% (29/54). There was no significant difference between bilirubin and hemoglobin values in HDN and non-HDN subjects. Conclusion The blood group of pregnant women with abnormal blood group of couples can predict the possibility of HDN in advance and reduce the incidence of HDN through prenatal treatment. Pregnancy has been detected in women who have had an unexplained abortion before pregnancy, and it is clear that miscarriage due to the incompatibility of the blood group of the husband and wife provides the basis for clinical treatment.
