近十多年来,计划生育、流产及产前检查门诊做宫颈涂片的青年增多,发现不少异常的细胞学。其中多人经阴道镜检町认出非典型上皮。据报告55%青年有乙酸变白宫颈上皮,其中22%有异常细胞,但异常区的活检仅为上皮化生。本文对一组青年妇女长期观察其宫颈阴道移行区的变化。共观察110人,平均年龄18岁(16~20),只检查盆腔器官,宫颈涂生理盐水、5%乙酸和碘液后作宫颈刮片。在阴道镜下从移行区取粘膜细胞标本做扫描电镜检查。乙酸变白上皮结合血管类型把移行区分为Ⅰ、Ⅱ、Ⅲ级。碘试验:阳性(无着色),可疑(部分着色),阴性(全着色)。每六个月检查一次。110人中71人(65%)的宫颈移行区有乙酸变白上皮,余39人(35%)则无。54人随访,平均14月后26人乙酸变白上皮消失变成充满糖原的成熟生化鳞状上皮。另28人至36月后仍未改变,内27人移行区限于宫颈,1人伸展到阴道。12人为柱状和乙酸变白混合上皮(2人有化生)。三年后10人为混合型移行区,18人仍为乙酸变白上皮。乙酸变白区大小不等:20人<宫颈表面的10%;5人占宫颈的
