目的比较分析噬菌体生物扩增(pha B)法、抗酸染色法和分枝杆菌培养法在结核病患者中的诊断性能,评价pha B法用于结核感染诊断的临床价值。方法采用pha B法、抗酸染色法和分枝杆菌培养法分别对2014年1月-12月就诊的157例结核病患者的晨痰和随机痰标本进行检测,以χ2检验比较其诊断性能的差异。结果 pha B法、抗酸染色法和分枝杆菌培养法对结核病患者的检测灵敏度分别为68.2%、52.2%和89.8%;以培养法结果为金标准,pha B法和抗酸染色法的阳性符合度分别为74.5%和57.4%(P<0.05),总体符合度为75.8%和60.5%(P<0.05);Pha B法在双份痰标本涂片法均为阴性的患者中的阳性检出率为33.3%(25/75);抗酸染色法的检测时间为1 h,Pha B法为46 h,培养法为9.5 d。结论 pha B法具有高灵敏度、高特异性、快速简便、可鉴别死菌或活菌、可进行耐药性检测等优点,适用于作为结核病的初筛试验或作为传统检测方法的有效补充试验。
Objective To compare and analyze the diagnostic value of pha B method, acid-fast staining method and mycobacterium culture method in tuberculosis patients and evaluate the clinical value of pha B method in the diagnosis of tuberculosis infection. Methods The sputum samples of sputum and sputum from 157 TB patients who were admitted to TB from January to December in 2014 were detected by pha B method, acid-fast staining method and mycobacterium culture method. The differences in diagnostic performance were compared by χ2 test . Results The detection sensitivity of pha B method, acid-fast staining method and mycobacterium culture method were 68.2%, 52.2% and 89.8% respectively. The results of culture method were the gold standard, pha B method and acid-fast staining method The coincidences were 74.5% and 57.4% respectively (P <0.05), and the overall coincidences were 75.8% and 60.5% respectively (P <0.05). The positive results of Pha B in patients with negative sputum smear The rate of 33.3% (25/75), the time of acid-fast staining was 1 h, the method of Pha B was 46 h, and the culture method was 9.5 d. Conclusion pha B method has the advantages of high sensitivity, high specificity, quick and easy, identification of dead or viable bacteria, drug resistance detection, etc., suitable for primary screening test as tuberculosis or as an effective supplementary test of traditional detection methods.
