目的:探讨阴茎折断合并尿道损伤的诊断与外科治疗方法及临床效果。方法:回顾性分析2003年4月~2012年6月收治的5例阴茎折断合并尿道损伤患者临床资料,总结诊断方法与治疗效果。结果:5例均急诊手术治疗,术后轻度包皮水肿2例,无切口感染、出血、尿瘘发生,留置尿管2W,拔管后无阴茎弯曲、海绵体白膜硬结及纤维化,无尿道狭窄等并发症发生。随访6个月~3年,患者术后6W后恢复性生活,阴茎勃起及功能满意,无阴茎弯曲、勃起疼痛、尿道狭窄等并发症出现。结论:阴茎折断合并尿道损伤根据典型病史及临床表现多可确诊,急诊经冠状沟环形切口修补疗效确切,并发症少,适于临床应用。 Objective: To investigate the diagnosis and surgical treatment of penile fracture combined with urethral injury and its clinical effect. Methods: The clinical data of 5 patients with penile fracture and urethral injury admitted from April 2003 to June 2012 were retrospectively analyzed, and the diagnostic methods and therapeutic effects were summarized. Results: All of the 5 cases were treated by emergency operation. There were 2 cases of mild foreskin edema after operation, no incision infection, bleeding, urinary fistula, indwelling catheter 2W, no penile curvature after extubation, cavernous alveolar induration and fibrosis, Complications such as stenosis occur. All the patients were followed up for 6 months to 3 years. The patients recovered after 6 weeks of operation, their penile erections and their functions were satisfactory. No complications such as penile curvature, erectile pain and urethral stricture occurred. Conclusions: The penile fracture combined with urethral injury can be diagnosed according to the typical history and clinical manifestations. The emergency treatment through the coronoid groove circular incision is effective and has few complications and is suitable for clinical application.
