目的：探讨提高下咽癌５年生存率及生存质量。方法：回顾性分析采用放射治疗、化疗、手术治疗及综合治疗２９０例下咽癌远期疗效。其中Ⅰ期３例，Ⅱ期１２例，Ⅲ期９６例，Ⅳ期１７９例；梨状窝癌２１０例，环后癌４２例，咽后壁癌３８例。单纯放射治疗１５例，放射治疗＋化疗２１例，诱导化疗＋手术４０例，单纯手术５３例，综合治疗（手术＋放射治疗＋化疗）１６１例。保留喉功能手术 １６３例，不保留喉功能 ９１例。结果：５年生存率分别为：单纯放射治疗２０．０％，放疗＋化疗２３．８％，单纯手术治疗４５．３％，诱导化疗＋手术６７．５％，综合治疗５６．５％，手术组保留喉功能的５年生存率为５７．６％，喉功能不保留组为５２．７％。结论：诱导化疗＋手术不仅提高生存率，而且提高喉功能保留率，综合治疗是提高下咽癌生存率有效方法，保留喉功能的下咽癌手术不影响生存率。
Objective: To explore to improve the 5-year survival rate and quality of life of hypopharyngeal carcinoma. Methods: A retrospective analysis of radiotherapy, chemotherapy, surgical treatment and comprehensive treatment of 290 cases of hypopharyngeal cancer long-term efficacy. There were 3 cases of stage Ⅰ, 12 cases of stage Ⅱ, 96 cases of stage Ⅲ and 179 cases of stage Ⅳ. There were 210 cases of pear-shaped nest tumor, 42 cases of posterior ring carcinoma and 38 cases of posterior pharyngeal wall cancer. Radiotherapy alone in 15 cases, radiotherapy + chemotherapy in 21 cases, induction chemotherapy + surgery in 40 cases, simple surgery in 53 cases, comprehensive treatment (surgery + radiotherapy + chemotherapy) in 161 cases. 163 cases of laryngeal surgery retained, 91 cases without preserving laryngeal function. Results: The 5-year survival rates were 20.0% for radiotherapy alone, 23.8% for radiotherapy and chemotherapy, 45.3% for simple surgery, 67.5% for induction chemotherapy and surgery, 56.5% for comprehensive treatment, The 5-year survival rate of the laryngeal function was 57.6% in the group and 52.7% in the laryngopharyngeal function group. Conclusion: Induction chemotherapy plus operation not only improves survival rate but also improves retention rate of laryngeal function. Comprehensive treatment is an effective method to improve the survival rate of hypopharyngeal carcinoma. The hypopharyngeal carcinoma with preserving laryngeal function does not affect the survival rate.
