目的了解养老机构老年人医疗服务利用状况,对不同医疗服务模式养老机构老年人医疗服务利用不足的原因进行比较分析。方法应用自行设计的问卷,采用多阶段分层抽样抽取山东省1 648名≥60岁老年人作为调查对象,运用二项logistic回归对导致不同医疗服务模式养老机构老年人医疗服务利用不足的原因进行分析。结果 “自我感觉没有大问题”和“经济困难”在老年人“患病未就诊”原因的分别占36.10%、13.53%。健康自评越好的老年人,由于“自我感觉没问题”不去就诊的概率越高;健康自评越差、过去职业为农民的老年人,由于经济困难而不去就诊的概率较高。医养分离模式的养老机构中,女性和婚姻状况为“非未婚”的老年人,由于“自我感觉没有大问题”不去就医的概率较高;在一体化模式的养老机构中,未婚的老年人“自我感觉没有大问题”导致不去就医的概率高。医养分离模式和外部合作模式的养老机构中,子女数越少的老年人,越可能因为经济困难不去就诊。结论促进老年人的自我健康意识,改善老年人的客观经济状况,增强老年人的支付能力,是提高老年人医疗服务利用的重要措施。
Objective To understand the utilization of medical services for the elderly in pension institutions and to analyze the reasons for the inadequate utilization of medical services for the elderly in pension institutions with different medical service modes. Methods A total of 1 648 elderly people aged 60 or older in Shandong Province were selected as the respondents by using self-designed questionnaire. Older persons with fewer children will be denied medical treatment because of financial difficulties among the pension institutions that provide medical separation and external cooperation. Conclusions Promoting self-awareness of the elderly, improving the objective economic conditions of the elderly and enhancing the ability of the elderly to pay are important measures to improve the utilization of medical services for the elderly.
