目的 探讨细胞角质蛋白19(CYFRA_(21-1))与癌胚抗原(CEA)在结核性胸水与癌性胸水中的诊断价值。方法胸水患者108例,其中癌性68例,结核性40例,分别测定其血清、胸水中CYFRA_(21-1)和CEA的浓度。结果①两种肿瘤标记物浓度在恶性胸水中明显高于结核性胸水;②癌性患者胸水中CYFRA_(21-1)的浓度明显高于血清中的浓度,而胸水中CEA浓度与血清中浓度的差异无显著性;③胸水CYFRA_(21-1)的检测存在一定的假阳性。结论 联合检测CYFRA_(21-1)与CEA对结核性与恶性胸水的诊断有较高的临床价值。
Objective To investigate the diagnostic value of cytokeratin 19 (CYFRA_ (21-1)) and carcinoembryonic antigen (CEA) in tuberculous pleural effusion and cancerous pleural effusion. Methods A total of 108 cases of pleural effusion, including 68 cases of cancerous and 40 cases of tuberculosis, were tested for CYFRA_ (21-1) and CEA concentrations in serum and pleural effusion respectively. Results ① The concentrations of both tumor markers in malignant pleural effusion were significantly higher than those in tuberculous pleural effusion. ② The concentration of CYFRA_ (21-1) in pleural effusion of cancerous patients was significantly higher than that in serum, whereas the concentration of CEA in pleural effusion and serum concentration The difference was not significant; ③ pleural effusion CYFRA_ (21-1) test there is some false positive. Conclusion Combined detection of CYFRA_ (21-1) and CEA has high clinical value in the diagnosis of tuberculous and malignant pleural effusion.
