目的通过观察宫颈癌超选择子宫动脉灌注化疗(介入化疗)后临床症状、肿瘤局部及超微结构动态变化,探讨超选择子宫动脉灌注化疗的疗效。方法对4例病理证实为宫颈鳞状细胞癌的病例,给予双侧子宫动脉灌注化疗,于介入前、后动态观察肿瘤局部及肿瘤细胞超微结构变化。结果 4例病例临床疗效均为部分缓解(PR)。超微结构改变为:(1)细胞间隙增宽,桥粒连接减少、结构发育差,微绒毛减少或消失;(2)细胞器改变:内质网肿胀形成囊泡、囊池,线粒体固缩、肿胀,溶酶体增多;细胞核固缩、溶解、破裂、异染色质增加、核浆比例缩小;(3)出现细胞肿胀、凋亡小体、凋亡细胞等凋亡形态表现;(4)周围细胞反应:炎细胞浸润及成纤维细胞较明显。结论超选择子宫动脉灌注化疗使宫颈肿瘤细胞超微结构改变,出现明显癌细胞凋亡现象,最终达到治疗目的 。
Objective To observe the clinical symptoms, ultrastructure and ultrastructure of cervical cancer after superselective uterine artery infusion chemotherapy (interventional chemotherapy) and investigate the curative effect of superselective uterine arterial infusion chemotherapy. Methods Four cases with cervical squamous cell carcinoma confirmed by histopathology were treated with bilateral uterine artery infusion chemotherapy. The ultrastructural changes of tumor and tumor cells were observed before and after intervention. Results The clinical efficacy of 4 cases were all partial response (PR). The ultrastructural changes were as follows: (1) widening of intercellular space, decrease of desmosome connection, poor structure development and decrease or disappearance of microvilli; (2) changes of organelles: swelling of endoplasmic reticulum to form vesicles, cysts, mitochondria shrinkage, Swelling and lysosome increase; pyknosis, dissolution and rupture of nucleus, increase of heterochromatin and decrease of nuclear cytoplasm; (3) appearance of apoptotic cells such as cell swelling, apoptotic bodies and apoptotic cells; (4) Cell response: inflammatory cell infiltration and fibroblasts more obvious. Conclusion Superselective uterine arterial infusion chemotherapy can change the ultrastructure of cervical cancer cells and cause obvious apoptosis of cancer cells, finally reaching the therapeutic goal.
