目的探索机械通气中困难撤机的对策。方法选择我科08年1月至09年12月机械通气中撤机困难患者70例,通过心理疏导,心功能调理,肺部感染控制,肺栓塞预防与治疗,呼吸肌泵功能训练,营养支持等干预措施,观察脱机成功率。结果 70例患者撤机成功59例,撤机成功率84.3%,成功撤机患者上机时间7~88d,平均16d。结论影响撤机的因素很多,应用多种方法祛除呼吸衰竭的病因,调控患者的生理功能,可提高撤机成功率。
Objective To explore the measures of mechanical weaning in weaning. Methods From January 2008 to December 2009, 70 cases of patients with weaning failure in mechanical ventilation were selected. Their psychological support, cardiac function, lung infection control, prevention and treatment of pulmonary embolism, functional training of respiratory muscle pump, nutritional support And other interventions, observe the success rate off-line. Results In the 70 cases, 59 cases were weeded out successfully, the success rate of weaning was 84.3%, and the successful weaning time was 7 ~ 88 days with an average of 16 days. Conclusion There are many factors that affect the weaning, and we can use a variety of methods to get rid of the cause of respiratory failure, regulate the physiological function of patients and improve the success rate of weaning.
