天津市肿瘤医院(原天津市人民医院)收治非手术综合治疗小细胞肺癌131例,其中男90例、女32例,中位年龄51岁,中位症状期3月,以咳、咯血痰为主。肺X片肺门阴影84.7％,肺野阴影13.7％,肺片阴性1.5％。支气管镜咬检阳性率94.7％、刷片阳性率69％,痰涂片阳性率72.6％。锁上淋巴结切检或针吸活检27例,皆阳性。本组可评定疗效者107例,其中局限型73例,广泛型34例。全组的中位生存期为12月,1年生存率为51.4％、3年14.1％和5年7.1％。本组男性预后稍优于女性,年龄越大预后越好,病期越长预后越差,局限型优于广泛型,化疗与放疗综合治疗优于单纯化疗,治疗越充分预后越好,锁骨上无淋巴结转移者优于有转移者,治疗反应达完全缓解者预后最好,部分缓解者次之,无缓解者最差,患者体质状态较好者优于体质状态较差者。
Tianjin Cancer Hospital (formerly Tianjin People’s Hospital) received 131 cases of non-surgical treatment of small cell lung cancer, including 90 males and 32 females. The median age was 51 years, and the median symptom was 3 months. The cough and hemoptysis were the Lord. Lung X-ray hilar shadow was 84.7%, lung field shadow was 13.7%, lung patch was negative 1.5%. The positive rate of bronchoscopic bite test was 94.7%, the positive rate of brush was 69%, and the positive rate of sputum smear was 72.6%. Locked lymph node examination or needle biopsy in 27 cases were positive. The group can evaluate 107 cases of efficacy, of which 73 cases of limited type, 34 cases of extensive. The median survival time for the whole group was 12 months, with a 1 year survival rate of 51.4%, 14.1% for 3 years, and 7.1% for 5 years. The prognosis of this group of men is slightly better than that of women. The older the prognosis is, the better the prognosis is. The longer the disease is, the worse the prognosis is. The limited type is better than the broad type. Comprehensive treatment of chemotherapy and radiotherapy is better than chemotherapy alone. The better the prognosis, the better the treatment is. Those with no lymph node metastasis were better than those with metastasis, and those with complete response to the treatment response had the best prognosis, followed by those with partial remission and those without remission, and those with better physical condition were better than those with poor physical status.
