目的:观察新疆哈萨克族、汉族原发性高血压患者血浆脂联素水平及其与胰岛素抵抗、血糖、血脂、血压及肥胖度指标的关系。方法:选取2005-05/10流行病学调查的新疆哈萨克族人127名及2005-09/2006-05在本院体检中心体检的汉族人131名为对象。根据血压水平,分为哈高血压组、哈对照组、汉高血压组、汉对照组。均取空腹静脉血,测定血浆脂联素、胰岛素、血糖、总胆固醇、三酰甘油、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇、载脂蛋白A1、载脂蛋白B及肥胖度指标等进行比较。结果:纳入的256参与者全部进入结果分析。①哈、汉高血压组的血浆脂联素水平均明显低于相应的健康对照组(P<0.01)。②哈、汉高血压组间比较或哈、汉健康对照组间血浆脂联素水平比较,差异无显著性意义(P>0.05)。③哈、汉女性血浆脂联素水平均高于男性。④在哈族高血压组中,脂联素与收缩压、舒张压、腰围、体质量指数、血糖、腰臀比、胰岛素、稳态模型计算胰岛素抵抗、三酰甘油、低密度脂蛋白胆固醇、载脂蛋白B均呈显著负相关。⑤在汉族高血压组中,脂联素与收缩压、舒张压、腰围、体质量指数、腰臀比、血糖、胰岛素、稳态模型计算胰岛素抵抗、三酰甘油亦均呈显著负相关,与载脂蛋白A1呈显著正相关。⑥调整年龄、血糖、血脂、肥胖指标等影响因素后,两民族血浆脂联素水平仍与收缩压、舒张压、胰岛素、稳态模型计算胰岛素抵抗存在显著负相关。结论:哈、汉原发性高血压患者血浆脂联素水平均降低并与胰岛素抵抗、血压、血糖、血脂、肥胖度指标存在一定的相关性。
Objective: To observe the plasma adiponectin level and its relationship with insulin resistance, blood glucose, blood lipid, blood pressure and obesity in Kazak and Han Chinese patients with essential hypertension in Xinjiang Uygur Autonomous Region. Methods: A total of 127 Kazakhs in Xinjiang from 2005-05 / 10 epidemiological survey and 131 Han Chinese from 2005-09 / 2006-05 were enrolled in this study. (5) In the Han hypertension group, adiponectin also showed a significant negative correlation with systolic blood pressure, diastolic blood pressure, waist circumference, body mass index, waist-hip ratio, blood glucose, insulin and insulin resistance, and triglyceride Apolipoprotein A1 was significantly and positively correlated. (6) After adjusting for age, blood glucose, blood fat, obesity index and other influencing factors, plasma adiponectin levels in both ethnic groups were still significantly negatively correlated with systolic blood pressure, diastolic blood pressure, insulin, and insulin resistance. Conclusion: Plasma adiponectin levels in patients with essential hypertension in both Kazakhstan and Han are decreased and have a certain correlation with insulin resistance, blood pressure, blood glucose, blood lipid and obesity index.
