目的前瞻性随机对照研究腺苷钴胺对嗅鞘细胞移植近期疗效的影响。方法2007年1月～2008年7月间,18例脑性瘫痪患者随机分为2组(A组为腺苷钴胺+嗅鞘细胞组,B组为单独嗅鞘细胞组),每组9人。两组性别、年龄、诊断分型、病程、智力情况、初始神经功能等分布无显著性差异。对嗅鞘细胞脑内移植术前术后患者的粗大运动功能评分(GMFM-66)、脑瘫调查问卷评分(caregiver questionnaire)及脑瘫日常生活活动能力(ADL)进行评价,其中GMFM-66和ADL分数越高代表功能状态越好、caregiver questionnaire分数越低越好。结果GMFM-66评分A组增加3.0±2.0分,B组增加2.0±2.0分(t=0.935,p=0.368),caregiver questionnaire评分A组降低3.7±5.0分,B组降低2.0±2.8分(t=0.795,p=0.442),ADL评分A组增加0.9±0.9分,B组增加0.4±0.7分(t=1.098,p=0.294)。结论对于脑性瘫痪的治疗,腺苷钴胺似能提高嗅鞘细胞移植治疗的近期临床效果,但因病例数尚少,统计学上未显示出差异的显著性,有待增加样本量,并对中长期效果探讨。 Objective To investigate the effect of adenosylcobalamin on the short term efficacy of olfactory ensheathing cell transplantation in a prospective randomized controlled trial. Methods From January 2007 to July 2008, 18 patients with cerebral palsy were randomly divided into 2 groups (group A was adenosylcobalamin + olfactory ensheathing cells, group B was olfactory ensheathing cells alone) people.
