目的探讨速度向量成像技术(VVI)在评价胎儿室壁心肌功能中的作用及其影响因素。方法通过Syngo US Workplace VVI工作站处理标准胎儿四腔心动态图(147例),分别获得左、右心室四腔心观心内膜VVI图,以及左、右心室纵向6个节段的速度、应变、应变率的测值并进行分析。结果 (1)左、右心室的速度分布由基底段向心尖逐渐减小(P<0.05),其6个节段和整体的应变、应变率的差异无统计学意义(P>0.05)。(2)左、右心室的速度和孕周呈正相关(左心室r=0.662,P<0.05;右心室r=0.732,P<0.05),整体应变、应变率和孕周无显著相关性(P>0.05)。(3)左、右心室整体应变率和胎心率呈正相关(左心室r=0.454,P<0.05;右心室r=0.465,P<0.05),其应变和速度均和胎心率无显著相关性(P>0.05)。结论 VVI技术可应用于胎儿整体和局部心肌运动和功能的分析评估。
Objective To investigate the role of velocity vector imaging (VVI) in the evaluation of myocardial function in fetal wall and its influencing factors. Methods Standard fetal four-chamber dynamic images (147 cases) were processed by Syngo US Workplace VVI workstation to obtain the left ventricle and the right ventricle, respectively. The left and right ventricular longitudinal six segments of the VVI, , Strain rate measurement and analysis. Results (1) Velocity distribution of left ventricle and right ventricle decreased gradually from basal segment to apex (P <0.05). There was no significant difference in strain and strain rate between six segments and whole segment (P> 0.05). (2) The velocities of left and right ventricle were positively correlated with gestational age (r = 0.662 in left ventricle, P <0.05; r = 0.732 in right ventricle, P <0.05). There was no significant correlation between overall strain and strain rate and gestational age > 0.05). (3) The left and right ventricular total strain rate and fetal heart rate were positively correlated (left ventricular r = 0.454, P <0.05; right ventricular r = 0.465, P <0.05), the strain and velocity were not correlated with fetal heart rate (P> 0.05). Conclusion The VVI technique can be applied to the assessment of fetal global and regional myocardial motion and function.
