目的探讨地特胰岛素对不同BMI的2型糖尿病患者胰高血糖素样肽-1(GLP-1)、瘦素(Leptin)和胃肠运动功能及促生长素(Ghrelin)水平影响。方法随机选取佛山市中医院2型糖尿病患者60例,根据体质量指数(BMI)分为正常BMI组和超重组,两组患者均起始加予地特胰岛素联合诺和龙治疗,1次/d,检测治疗前和治疗2年后GLP-1、Leptin和Ghrelin的变化水平。结果地特胰岛素治疗后两组Leptin和GLP-1水平升高,Ghrelin水平降低,与治疗前比较差异均有统计学意义(P<0.05),其中超重组Leptin和GLP-1水平升高更为明显,与正常BMI组和治疗前比较差异均有统计学意义(P<0.05),但两组Ghrelin水平比差异无统计学意义(P>0.05),两组患者治疗后BMI比较差异无统计学意义(P>0.05)。结论地特胰岛素对2型糖尿病患者体内食欲相关激素的影响较为明显,其在减少2型糖尿病患者体质量增加中具有显著作用。
Objective To investigate the effect of insulin detemir on the levels of GLP-1, Leptin and gastrointestinal motility and Ghrelin in type 2 diabetic patients with different BMI. Methods Sixty patients with type 2 diabetes mellitus were randomly selected in Foshan Hospital of Traditional Chinese Medicine. Patients were divided into normal BMI group and overweight group according to body mass index (BMI). Patients in both groups were treated with detemir plus noravone once a day, d, To detect the changes of GLP-1, Leptin and Ghrelin before treatment and 2 years after treatment. Results After insulin treatment, Leptin and GLP-1 levels were increased and Ghrelin levels were lower in both groups (P <0.05), and the levels of Leptin and GLP-1 in overweight group were more Obviously, compared with normal BMI group and before treatment, the differences were statistically significant (P <0.05), but there was no significant difference in Ghrelin level between the two groups (P> 0.05). There was no significant difference in BMI between the two groups Significance (P> 0.05). Conclusion Dextrins have an obvious effect on appetite-related hormones in patients with type 2 diabetes mellitus, which play a significant role in reducing the body mass of patients with type 2 diabetes mellitus.
