目的:研究体外肝叶切除自体余肝原位再植术对机体氧供需平衡的影响。方法:7 只健康幼猪,麻醉后分别经脾静脉插管至门静脉和右股静脉插管至下腔静脉,两管经 Y 型管联接,右颈内静脉插管至右心房,全肝血流阻断时建立门腔心房体外辅助循环。在麻醉后、转流30 min 及转流后30 min 分别测定血流动力学指标、混合静脉血氧饱和度( S珔v O2) 及血气分析。结果:①术中 S珔v O2 显著降低( P < 0 .05) ;②静脉转流能部分纠正术中血流动力学紊乱;③心输出量与 S珔v O2 存在相关关系。结论:提示体外肝叶切除自体余肝原位再植术中存在氧供需状态的失衡。
OBJECTIVE: To study the effect of in situ replantation of autogenous residual liver on the oxygen supply and demand balance in vitro. METHODS: Seven healthy piglets were anesthetized and were cannulated through the splenic vein to the portal vein and right femoral vein and cannulated to the inferior vena cava. The two tubes were connected via a Y-tube and the right internal jugular vein was cannulated to the right atrium. Whole liver blood was obtained. When the flow is blocked, the door cavity-atrial extracorporeal circulation is established. Hemodynamic parameters, mixed venous oxygen saturation (S珔v O2), and blood gas analysis were measured after anesthesia, 30 minutes after the transition, and 30 minutes after the transition. RESULTS: 1 During operation, S珔v O2 decreased significantly (P < 0.05); 2 venous inflow partially corrected intraoperative hemodynamic disturbance; 3 cardiac output correlated with S珔v O2. Conclusion: There is an imbalance of oxygen supply and demand during liver resection of autogenous residual liver in vitro.
