Increased middle ear pressure during general anesthesia with nitrous oxide and oxygen may be the major contributor to many complications, such as temporary or permanent hearing loss, tympanic hemorrhage, tarsal rupture, rupture of the tympanic membrane, severe otitis media and transposition of the tympanic membrane the reason. The authors report a case of nitrous oxide general anesthesia caused tympanic membrane rupture. This case is a 46-year-old woman, her ear often fluid, due to renal atrophy caused by hypertension, to be planned right nephrectomy. Preoperative chief complaint of hearing loss, occasional tinnitus and cold pain. According to their age, when the frequency of 125Hz ~ 8000Hz, hearing should be within the normal range. Middle ear pressure was measured with an impedance bridge on the left ear, two high frequency trials (200 Hz and 660 Hz) were performed, and a high amplitude tympanic pressure diagram was presented.
