目的 探讨脑苷肌肽辅助治疗对急性脑梗死患者血脑屏障、神经功能及脑水肿发生的影响.方法 选择本院收治的80 例急性脑梗死患者, 按随机数字表法分为对照组与观察组各40 例.两组均给予常规治疗, 观察组在此基础上加用脑苷肌肽, 记录临床疗效, 测定治疗前、治疗2 周后患者血清基质金属蛋白酶7 (MMP-7) 、基质金属蛋白酶8 (MMP-8) 、 S100B 蛋白(S100B)等血脑屏障指标的变化, 采用神经功能缺损量表(NIHSS)、生活质量评价表(QOL100)评定患者神经功能及生活质量, 并作颅脑CT检查, 统计患者脑水肿发生率, 并作脑水肿分级.结果 ①观察组治疗总有效率高于对照组(P<0.05) ;②治疗2 周, 两组 MMP -7、 MMP -8、 S100B 水平均降低, 与治疗前对比差异有统计学意义(P<0.05) , 观察组降低幅度高于对照组(P<0.05) ;③治疗2 周, 两组NIHSS 表评分降低, QOL100 表评分上升, 与治疗前对比差异有统计学意义(P <0.05) , 观察组 NIHSS 表评分降低及 QOL100 表评分上升幅度均高于对照组(P<0.05);④观察组治疗2 周脑水肿发生率低于对照组(P <0.05).结论 急性脑梗死患者在常规对症处理基础上加用脑苷肌肽辅助治疗, 可改善患者神经功能, 减轻脑水肿, 保护患者血脑屏障.“,”Objective To investigate the effect of cattle encephalon glycoside and ignotin injection on the blood-brain bar-rier, neurological function and incidence of brain edema in patients with acute cerebral infarction.Methods 80 patients with acute cerebral infarction were divided into the control group and the observation group according to random number table.Both groups were given conventional treatment, and the observation group was additionally treated with cattle encephalon glycoside and ignotin injection.After 2 weeks of treatment, serum levels of MMP-7, MMP-8 and S100B were measured to determine the integrity of blood-brain barrier.NIHSS and QOL100 scale were used to evaluate the neurological function and quality of life.CT scan was performed to detect the cerebral edema.Results The overall response rate in the observation group was higher than that in the control group (P <0.05).The levels of MMP-7, MMP-8 and S100B after treatment were lower than those before treatment, and the levels in the ob-servation group was significant lower than those in the control group (P<0.05).NIHSS scores were significantly decreased (P<0.05), while QOL100 scores were significantly increased after treatment than those before treatment (P <0.05), the changes of scores in the observation group were more significant than those in the control group (P<0.05).The incidence rate of cerebral edema in the observation group was lower than that in the control group (P<0.05).Conclusion The application of conventional therapy combined with cattle encephalon glycoside and ignotin injection can improve neurological function, relieve cerebral edema and protect blood-brain barrier in patients with acute cerebral infarction.
