目的 探讨螺旋CT多平面和曲面重建成像对面神经管显示的临床应用。方法 对1具尸体头颅标本、2个颅骨标本及 4 3例患者进行颞骨螺旋高分辨CT扫描 ,层厚 1 0mm、间距0 2mm、螺距为 1。然后进行冠状面、矢状面及斜面的多平面重建 (MPR)和面神经管全程的曲面重建成像 ,对尸体标本解剖面神经管 ,并与其断面图像对比。结果 CT图像所见与标本解剖一致 ,4 0侧正常耳的面神经管迷路段、水平段在轴面扫描图像上显示最优 ,面神经管垂直段在矢状面及斜面重建图像上显示最优 ,曲面重建能显示面神经管连续的全程。中耳乳突炎 2 5耳中 ,面神经管水平段及垂直段有破坏者分别为 6耳及 5耳 ;先天性外耳道闭锁的 16耳中 ,面神经管异常达 10耳 ;外伤后面瘫的 5耳中 ,3耳见有面神经管骨折。面神经管在螺旋CT多平面和曲面重建图像上的表现与临床手术所见相符。结论 螺旋CT多平面和曲面重建成像技术有利于面神经管的显示及其病变的诊断。
Objective To investigate the clinical application of spiral CT multiplanar and curved reconstruction imaging of facial nerve tube. Methods One corpse skull specimen, two skull specimens and 43 patients underwent temporal bone spiral CT scan. The thickness was 10 mm, the pitch was 0 2 mm and the pitch was 1. Then, coronal, sagittal and bevel multiplanar reconstruction (MPR) and facial nerve canal reconstruction were performed on the whole surface of the neural tube of the autopsy specimens and compared with the cross-sectional images. Results The findings of CT images were consistent with the anatomy of the specimens. The lost segments and horizontal segments of the facial nerve canal in 40 normal ears showed the best results in the axial scanning images. The vertical segments of the facial neural tube showed the best in the sagittal and oblique reconstruction images, Surface reconstruction can show the continuity of the facial nerve tube. Middle ear otitis media 2 5 ears, facial nerve canal horizontal segment and the vertical segment of destructive were 6 ears and 5 ears; congenital external auditory canal atresia of 16 ears, facial nerve tube abnormalities up to 10 ears; traumatic paraplegia after 5 ears In the 3 ears see facial nerve tube fracture. Facial nerve canal in spiral CT multiplanar and curved reconstruction of the performance of the image and clinical findings. Conclusion Spiral CT multiplanar and curved reconstruction imaging is beneficial to the display of facial nerve canal and the diagnosis of its lesions.
