目的:探讨主动呼吸控制(active breathing coordination,ABC)技术在肺癌精确放疗中的应用。方法:选取2006~2007年间来我科的12例肺癌患者,使用ABC技术联合三维适形放疗技术治疗,CT定位扫描时分别采集ABC和自由呼吸(free breath,FB)两套图像,计算比较两者胸壁动度、膈肌动度、大体靶区(gross tumor volume,GTV)、临床靶区(clinnical target volume,CTV)、计划靶区(planning target volume,PTV)及双肺V20、Dmean。结果:ABC状态下胸壁动度、膈肌动度、GTV、CTV、PTV及双肺V20、Dmean比FB状态下均要小,且差异有统计学意义(P<0.05)。结论:在肺癌的精确放射治疗中,ABC系统可以有效的降低呼吸运动对治疗的影响,提高放疗的精确性,减少副反应。
Objective: To explore the application of active breathing coordination (ABC) technique in radiotherapy of lung cancer. Methods: Twelve patients with lung cancer who came to our department from 2006 to 2007 were selected and treated with ABC technique combined with three-dimensional conformal radiotherapy. Two images of ABC and free breath (FB) were collected during CT scanning. The results were as follows: chest wall mobility, diastolic motion, gross tumor volume (GTV), clinical target volume (CTV), planning target volume (PTV) and bilateral lung V20 and Dmean. Results: Under the condition of ABC, the dynamic of thoracic wall, diastolic motion of diaphragm, GTV, CTV, PTV, V20 and Dmean of both lungs were all smaller than those in FB state, and the difference was statistically significant (P <0.05). Conclusion: In the accurate radiotherapy of lung cancer, ABC system can effectively reduce the impact of respiratory motion on the treatment, improve the accuracy of radiotherapy and reduce side effects.
