目的探讨Merkel细胞癌的临床病理特征、诊断和鉴别诊断、治疗及预后。方法回顾分析4例Merkel细胞癌患者的临床资料、组织病理形态和免疫组化结果。结果光镜下癌细胞呈巢索状弥漫排列,形态类圆形、空泡状,大小较一致,染色质呈细颗粒状。免疫组化示AE1/AE3、CK20、EMA、NSE、CgA、Syn和CD56(+)。结论Merkel细胞癌是一种高度恶性的皮肤原发性神经内分泌癌,掌握临床病理及免疫组化特征对该病的诊断和鉴别诊断具有重要意义。彻底切除病灶结合术后放疗或化疗是治疗Merkel细胞癌的主要手段。
Objective To investigate the clinical and pathological features, diagnosis, differential diagnosis, treatment and prognosis of Merkel cell carcinoma. Methods The clinical data, histopathology and immunohistochemistry of 4 Merkel cell carcinoma patients were retrospectively analyzed. Results The cancer cells in the light microscope were arranged in a nest-like pattern with round and vacuolistic shapes and the same size. The chromatin was finely grained. Immunohistochemistry showed AE1 / AE3, CK20, EMA, NSE, CgA, Syn and CD56 (+). Conclusions Merkel cell carcinoma is a highly malignant primary neuroendocrine carcinoma of the skin. It is of great importance to diagnose and differentiate the clinical pathology and immunohistochemical features of the disease. Radical resection combined with postoperative radiotherapy or chemotherapy is the main means of treatment of Merkel cell carcinoma.
