在高脂蛋白血症的诊断和定型过程中,主要依据各种脂蛋白的数据并参考血脂值。多数情况下,血清胆固醇和甘油三酯分别与β-脂蛋白和前β-脂蛋白一致。对于少数特殊病例的分型,本文根据 WHO 表型分类提出了某些修正和补充。测定了58例冠心病和119例高血压病患者的血脂和脂蛋白。发现胆固醇的增高主要来自β-脂蛋白而不象高令健康人主要来自α-脂蛋白,甘油三酯的增高主要来自前β-脂蛋白。冠心病组中伴有高脂蛋白血症患者占74%,为正常组5.9倍;高血压病组中则占43.6%,为正常组3.5倍。从高脂蛋白血症的分型来看,冠心病组与高血压病组几乎全层Ⅱ型或Ⅳ型,冠心病组中Ⅱa,Ⅱb及Ⅳ型分别占23.3%,32.6%及44.1%,高血压病组则相应为11.5%,48.1%及38.5%,仅1例为Ⅲ型,占1.9%。冠心病组中胆固醇及甘油三酯含量的频数分布与正常组的重叠均小于高血压病组的重叠。以上结果说明高脂蛋白血症Ⅱ型与Ⅳ型肠患心血管疾患,因此测定血脂和脂蛋白对予示、辅助诊断及防止冠心病和高血压病具有一定忌义,其中对冠心病价值较大。
