目的调查分析2型糖尿病患者心理状况,探讨实施心理干预及抗抑郁药物治疗的疗效。方法 2012年5月1日至2014年3月31日纳入中山市小榄镇2型糖尿病患者250例为研究对象,采用流行病学现状调查对患者进行问卷调查,心理科医生及社区卫生服务站医生发放调查问卷,行体格检查,给予心理评估。根据分析结果,挑选SAS、SDS、SCL-90评分>40分患者(多虑平组)给予心理干预及多虑平治疗,对比观察两组患者治疗前后的心理状况及血糖控制情况。结果经调查发现,多虑平组患者FPG、PBS、Hb A1C、SDS、SAS、SCL-90结果均显著高于对照组,差异均有统计学意义(P<0.01);且治疗后SDS、SAS、SCL-90评分及FPG、PBS、Hb A1C水平均较治疗前显著降低(P<0.05)。结论多虑平结合心理干预对伴有焦虑抑郁的糖尿病患者疗效显著,可明显改善患者负性心理,提高血糖控制效果。
Objective To investigate the psychological status of patients with type 2 diabetes mellitus and to explore the effect of psychological intervention and antidepressant drug therapy. Methods From May 1, 2012 to March 31, 2014, 250 patients with type 2 diabetes mellitus in Xiaolan Town, Zhongshan City were enrolled in this study. Patients were surveyed by epidemiological status quo survey. Psychological doctors and community health service stations The doctor issued a questionnaire, physical examination, to give psychological assessment. According to the results of the analysis, the psychological intervention and doxepin treatment were selected in the patients with SAS, SDS, SCL-90 score> 40 (doxepin group), and the psychological status and the control of blood glucose before and after treatment were compared between the two groups. Results After investigation, the results of FPG, PBS, Hb A1C, SDS, SAS and SCL-90 in the doxepin group were significantly higher than those in the control group (P <0.01) , SCL-90 score, FPG, PBS and Hb A1C levels were significantly lower than those before treatment (P <0.05). Conclusion The combination of doxepin and psychological intervention has significant curative effect on diabetic patients with anxiety and depression, which can significantly improve the negative psychology of patients and improve the effect of blood sugar control.
