目的探讨个性化干预策略对预防脊柱骨折患者呼吸机相关性肺炎(VAP)的影响。方法选取2015年2月—2016年2月入住ICU行有创机械通气、时间超过48h的患者86例,分对照组(常规护理43例)和干预组(个性化护理43例),观察并记录二组VAP发生率、机械通气时间、ICU住院时间并进行比较。结果干预组VAP发病率、机械通气时间、ICU住院时间均较对照组低,且差异均有统计学意义(P<0.05);经治疗并撤离机械通气1h后,动脉血气分析干预组较对照组有明显改善,且差异均有统计学意义(P<0.05)。结论在患者机械通气治疗期间,实施有针对性的个性化护理干预措施,有助于缩短机械通气时间,降低呼吸机相关性肺炎发生率,提高治疗效果,具有明显的临床意义,值得临床推广。
Objective To investigate the effect of personalized intervention on the prevention of ventilator-associated pneumonia (VAP) in patients with spinal fractures. Methods Totally 86 patients who underwent invasive mechanical ventilation for more than 48 hours in ICU from February 2015 to February 2016 were divided into control group (43 cases) and intervention group (43 cases), and observed and recorded Two groups of VAP incidence, mechanical ventilation time, ICU hospital stay and compared. Results The incidence of VAP, duration of mechanical ventilation and ICU stay in the intervention group were lower than those in the control group (P <0.05). After treatment and withdrawal of mechanical ventilation for 1 hour, the arterial blood gas analysis intervention group was significantly lower than the control group Significantly improved, and the differences were statistically significant (P <0.05). Conclusions The implementation of targeted individualized nursing intervention during the mechanical ventilation therapy in patients can shorten the duration of mechanical ventilation, reduce the incidence of ventilator-associated pneumonia and improve the therapeutic effect. It has obvious clinical significance and is worthy of clinical promotion.
