目的探讨煤工尘肺(CWP)患者合并糖尿病(DM)时肺功能的变化。方法本研究设CWP合并DM组(50例)为观察组,单纯CWP组(50例)为对照组。将CWP合并DM组根据有无微血管病变再分为无并发症组(33例)和有并发症组(17例),检测用力肺活量(FVC)、第一秒用力呼气容积(FEV1)、第一秒用力呼气容积占用力肺活量的百分比(FEV1/FVC)、氧分压(PaO2)、二氧化碳分压(PaCO2)等肺功能指标,进行统计学处理,分别比较单纯CWP组、CWP合并DM组以及CWP合并糖尿病有、无并发症组之间各项肺功能指标的差异。结果 CWP合并DM组FVC、FEV1、PaO2明显低于单纯CWP组,差异有统计学意义(P<0.01),CWP合并DM有并发症组FVC、FEV1低于CWP合并糖尿病无并发症组,差异有统计学意义(P<0.01),三组间FFEV1/FVC、PaCO2差异无统计学意义。结论 CWP患者合并DM时肺功能进一步受到损伤,在伴有糖尿病微血管病变时,将进一步加重限制性通气功能障碍。
Objective To investigate the changes of lung function in coal worker’s pneumoconiosis (CWP) patients with diabetes mellitus (DM). Methods In this study, CWP combined DM group (50 cases) as the observation group, simple CWP group (50 cases) as the control group. The CWP combined with DM group was further divided into non-complication group (33 cases) and complications group (17 cases) according to the presence or absence of microvascular disease. FVC, FEV1, The FEV1 / FVC, PaO2, PaCO2 and other pulmonary function parameters were measured and statistically analyzed. The levels of FEV1 / FVC, CWP combined with DM And CWP with diabetes, non-complication group differences in various lung function indicators. Results The FVC, FEV1 and PaO2 in CWP combined with DM group were significantly lower than those in CWP group (P <0.01). The CWP combined DM group had complications with FVC and FEV1 was lower than that without CWP complicated with diabetes without complications Statistical significance (P <0.01), FFEV1 / FVC, PaCO2 differences between the three groups was not statistically significant. Conclusions Pulmonary function is further impaired in CWP patients with DM, which further aggravates restrictive ventilatory dysfunction when accompanied with diabetic microangiopathy.
