目的分析彩色多普勒超声在监测重度子痫前期患者胎儿脐动脉、大脑中动脉血流动力学中的应用价值。方法纳入2015年10月-2016年10月在广州市花都区妇幼保健院分娩的重度子痫前期的50例剖宫产孕妇作为病例组,另选取50例既往体健,无内外科合并症的剖宫产孕妇作为一般组,应用彩色多普勒超声联合监测两组研究对象胎儿脐动脉、大脑中动脉血流动力学,比较两组血流动力学参数指标血流阻力指数(RI)、搏动指数(PI)、舒张末期血流速度的比值(S/D)。结果病例组患者胎儿脐动脉RI、PI及S/D值均高于一般组(P<0.05),大脑中动脉RI、PI及S/D值均低于一般组(P<0.05),新生儿Apgar评分为(6.5±0.8)分,低于一般组(8.3±0.5)分(P<0.05),病例组新生儿脐动脉血气正常率为76.0%,低于一般新生儿的96.0%(P<0.05)。结论根据彩色多普勒超声监测重度子痫前期患者胎儿脐动脉、大脑中动脉结果,能够对胎儿宫内缺氧状况进行评估,并能为临床制定干预治疗方案提供参考依据。
Objective To analyze the value of color Doppler ultrasound in the monitoring of umbilical artery and middle cerebral artery hemodynamics in patients with severe preeclampsia. Methods Fifty cases of severe preeclampsia cesarean section delivered from October 2015 to October 2016 in Huadu District Maternity and Child Care Hospital of Guangzhou were selected as the case group and another 50 cases of previous physical health and no surgical complications Of cesarean section pregnant women as a general group, the application of color Doppler ultrasound monitoring of the two groups of fetal umbilical artery, middle cerebral artery hemodynamics, blood flow parameters of two groups were compared flow resistance index (RI) Pulsatile index (PI), end-diastolic blood flow velocity ratio (S / D). Results The RI, PI and S / D values of fetal umbilical artery were significantly higher in case group than those in general group (P <0.05). RI, PI and S / D in middle cerebral artery were lower than those in general group (P <0.05) The Apgar score was (6.5 ± 0.8) points lower than the normal group (8.3 ± 0.5) points (P0.05). The normal rate of neonatal umbilical arterial blood gas was 76.0% in neonates, which was lower than 96.0% of normal neonates (P < 0.05). Conclusion The results of fetal umbilical artery and middle cerebral artery can be used to evaluate fetal hypoxia status based on color Doppler ultrasound in patients with severe preeclampsia and to provide a reference for clinical intervention programs.
