目的 探讨黄体囊肿出血的三维超声图像特点.方法 对48例(53枚)黄体囊肿出血进行三维超声及三维能量多普勒超声成像,观察肿物的内外部特征、血供分布及血流动力学参数[搏动指数(PI)、阻力指数(RI)、最大峰值流速(PSV)].病例均经手术病理或长期随访证实.结果 根据三维超声图像,可将48例(53枚)黄体囊肿出血分为点状回声型(24枚)、筛网状回声型(9枚)、不均质高回声型(12枚)和不均质低回声型(8枚)4种;根据三维能量多普勒超声图像,将黄体囊肿出血分为无血流信号型(21枚)、点线状血流信号型(12枚)和球面状血流信号型(20枚)3种.32枚肿块显示出血流信号,筛网状回声型黄体囊肿出血患者的PI(0.71±0.01)及PSV[(32.0±1.0)cm/s]高于点状回声型、不均质高回声型和不均质低回声型,RI(0.50±0.03)低于其他3型,差异均有统计学意义(P<0.05).结论 三维超声能立体、直观地观察肿物的内外部微细结构及血流信号分布,结合三维能量多普勒技术能获得更多的细节信息,为黄体囊肿出血的诊断提供更多形态学依据.“,”Objective To discuss the features of three-dimension ultrasound(3DUS) and three-dimensional power Doppler ultrasound(3DPD)of hemorrhagic corpus luteum cyst(HCLC).Methods Forty-eight HCLC patients (53 cysts) were given 3DUS and 3DPD to observe the internal or external characteristics of cysts,distribution of blood supply,hemodynamic parameters including pulsatility index (PI),resistive index (RI),maximal peak velocity (PSV).All patients were conformed by surgical pathology or long-term follow-up.Results Based on 3DUS,53 cysts (48 patients) were classified into types spot echo(24 cysts),mesh-like echo(9 cysts),non-homogeneous high echo(12 cysts) and low echo(8 cysts);based on 3DPD,cysts classified into types asanguineous signal(21 cysts),dot-linear-shaped flow signal(12),spherical-shaped(20 cysts).Thirty-two cysts showed hemorrhagic flow signal,and PI and PSV were higher in patients with mesh-like echo type (0.71±0.01,32.0±1.0 cm/s) than in patients with spot echo,with non-homogeneous high echo or with low echo,but RI was lower (0.50±0.03) than other three groups,the difference was significant(P<0.05).Conclusion 3DUS can provide reliable information for HCLC diagnoses.The signal of spherical bloodstream is the important diagnostic basis for HCLC.
