1 病例rn患者,男,60岁.2013年7月16日以突发复视4d入院.入院前4d患者晨起时突发双眼视物成双,且每日晨起时较重,午后症状相对缓解.既往有高血压病史10年,血压在140/90 mmHg左右,发现“血糖升高”2周;2007年因“青光眼”、“左眼管状视野”双眼行手术治疗(具体术式不详).入院内科查体:心肺腹查体未见异常.神经系统查体:神志清楚,言语流利;左眼管状视野,双侧瞳孔不规则(手术后状态),直径约3 mm,对光反射灵敏,右眼外下方运动受限,余各向运动尚可.余神经系统查体未见异常.入院监测空腹血糖在10 mmol/L左右,餐后2h血糖波动于14～20mmol/L.头颅MRI+ MRA均未见异常.
