为了研制阿霉素明胶微粒并探讨用其化学栓塞治疗骨肉瘤的可行性和临床病理学变化特点,采用物理机械方法制出阿霉素明胶微粒并用此对26例骨肉瘤施行超选择瘤血管栓塞,对其中15例手术者进行详尽的临床及病理学观察,结果:①自制阿霉素明胶微粒方法简单可行,用此栓塞骨肉瘤有化学栓塞及末梢栓塞的作用,栓塞初期,栓塞综合征较明显,2周后疼痛消失,瘤体明显缩小,质地变硬,皮温可降至正常,术中发现瘤假包膜明显增厚,瘤体边界清楚,出血显著减少,无并发症。②组织大切片观察见瘤体大部分坏死,平均有效细胞坏死率>80%,与穿刺标本比较有显著性差异(P<0.01)。因此,采用阿霉素明胶微粒进行骨肉瘤化学栓塞是保肢手术前1种重要且有效的辅助治疗手段。
In order to develop adriamycin-modified gelatin microparticles and investigate the feasibility and clinical pathological changes of osteosarcoma treated with chemical embolization, a physical and mechanical method was used to prepare adriamycin-containing gelatin microparticles and superselective tumor vascular embolization was performed on 26 osteosarcomas. The detailed clinical and pathological observations were performed on 15 of them. Results: 1 Self-made gelatin microparticles of adriamycin were simple and feasible. The embolization of osteosarcoma had the effects of chemical embolization and peripheral embolization. In the early stage of embolization, embolism syndrome was more common. Obviously, the pain disappeared after 2 weeks, the tumor was significantly reduced, the texture became hard, the skin temperature could be reduced to normal, and the tumor pseudocapsule was found to be significantly thickened during the operation. The boundary of the tumor was clear, and the bleeding was significantly reduced without complications. 2 Large sections of tissue showed necrosis of tumors with an average effective cell necrosis rate of >80%, which was significantly different from that of puncture specimens (P<0.01). Therefore, the use of adriamycin-coated gelatin microparticles for osteosarcoma chemoembolization is an important and effective adjuvant therapy before limb salvage operation.
