目的探讨超声弹性成像联合常规超声鉴别乳腺良恶性肿瘤的价值。方法 132例乳腺肿瘤患者(共153个病灶)术前均行超声弹性成像和常规超声检查,以病理诊断为金标准,计算常规超声、超声弹性成像、两种超声联合诊断及应变率比值法诊断乳腺良恶性肿瘤的灵敏度、特异度、准确度及受试者工作特征(ROC)曲线下面积。结果 153个病灶中良性92个,恶性61个。联合诊断灵敏度高于常规超声、超声弹性成像和应变率比值法(90.16%vs.72.13%、81.97%和78.69%)(P<0.05),超声弹性成像特异度和准确度与其他三种方法比较无统计学差异(P>0.05)。超声弹性成像、联合诊断、应变率比值法ROC曲线下面积均大于常规超声(0.898、0.837、0.902vs.0.798)(P<0.05)。结论超声弹性成像联合常规超声有助于乳腺良恶性肿瘤的鉴别诊断。
Objective To investigate the value of ultrasound elastography combined with conventional ultrasound in differential diagnosis of benign and malignant breast tumors. Methods Thirty-two patients with breast cancer (153 lesions) underwent preoperative ultrasound elastography and routine ultrasound examination. The pathological diagnosis was used as the gold standard. Conventional ultrasound and elastography were performed. The diagnostic results of two kinds of ultrasound and strain rate ratio Sensitivity, specificity, accuracy, and area under the receiver operating characteristic (ROC) curve of breast benign and malignant tumors. Results Of the 153 lesions, 92 were benign and 61 were malignant. The sensitivity of combined diagnosis was higher than that of conventional ultrasound, ultrasound elastography and strain rate ratio (90.16% vs.72.13%, 81.97%, 78.69%) (P <0.05). The specificity and accuracy of ultrasound elastography were compared with those of the other three methods No statistical difference (P> 0.05). The area under the ROC curve of ultrasound elastography, combined diagnosis and strain rate ratio were all higher than that of conventional ultrasound (0.898,0.837,0.902 vs. 0.788) (P <0.05). Conclusion Ultrasound elastography combined with conventional ultrasound is helpful for the differential diagnosis of benign and malignant breast tumors.
