目的评价维持性血液透析患者钙磷代谢紊乱经血液透析联合血液灌流治疗的效果。方法选择我院2014年4月~2015年4月收治的78例维持性血液透析伴钙磷代谢紊乱患者为研究对象,根据治疗方法的不同进行分组,对照组39例患者进行血液透析治疗;试验组39例进行血液透析联合血液灌流治疗。比较两组患者治疗前后血磷、血钙、钙磷乘积、血肌酐(SCr)、尿素氮、全段甲状旁腺激素(i PTH)、骨钙素(BGP)含量。结果治疗6个月后,试验组患者血磷离子浓度为(1.97±0.35)mmol/L、钙磷乘积为(54.51±10.01)mmol2/L2、SCr为(719.27±15.21)μmol/L、尿素氮为(17.28±2.17)g/L,均明显低于对照组的(2.21±1.34)mmol/L、(62.21±10.46)mmol2/L2、(826.28±80.64)μmol/L、(18.39±1.17)g/L,而钙离子含量为(2.58±1.00)mmol/L,明显高于对照组的(2.11±0.19)mmol/L,差异有统计学意义(均P<0.05);试验组患者BGP含量为(251.67±35.14)μg/L、i PTH为(978.73±200.33)mmg/L,明显低于对照组的(279.57±25.46)μg/L、(1100.01±190.17)mmg/L,差异有统计学意义(均P<0.05)。结论血液透析联合血液灌流治疗维持性血液透析伴钙磷代谢紊乱,患者体内钙离子、磷离子、钙磷乘积、SCr、尿素氮、BGP、i PTH等实验室指标含量均有明显改善,短期临床效果较好。
Objective To evaluate the effect of hemodialysis combined with hemoperfusion on disorders of calcium and phosphorus metabolism in maintenance hemodialysis patients. Methods A total of 78 patients with maintenance hemodialysis with metabolic disorder of calcium and phosphorous in our hospital from April 2014 to April 2015 were enrolled in this study. According to the different treatment methods, 39 patients in the control group were treated with hemodialysis. Thirty-nine patients underwent hemodialysis combined with hemoperfusion. Blood phosphorus, serum calcium, calcium and phosphorus products, serum creatinine (SCr), blood urea nitrogen, total parathyroid hormone (i PTH) and BGP were compared between the two groups before and after treatment. Results Six months after treatment, the concentrations of phosphorus and phosphorus in the test group were (1.97 ± 0.35) mmol / L, (54.51 ± 10.01) mmol / L and SCr were (719.27 ± 15.21) μmol / L and (2.21 ± 1.34) mmol / L, (62.21 ± 10.46) mmol / L, (826.28 ± 80.64) μmol / L and (18.39 ± 1.17) g / L, while the content of calcium was (2.58 ± 1.00) mmol / L, which was significantly higher than that of the control group (2.11 ± 0.19) mmol / L (all P <0.05) (251.67 ± 35.14) μg / L, i PTH was (978.73 ± 200.33) mmg / L, which was significantly lower than that of the control group (279.57 ± 25.46 μg / L, 1100.01 ± 190.17 mmg / L, (All P <0.05). Conclusion Hemodialysis combined with hemoperfusion can maintain the hemodialysis with disorder of metabolism of calcium and phosphorus. The contents of calcium, phosphorus, calcium and phosphorus, SCr, urea nitrogen, BGP, i PTH and other lab indexes in patients with hemodialysis were significantly improved.
