根据尿蛋白分子的大小可区别肾小球病变、肾小管病变或者是生理性蛋白尿。尿蛋白聚丙烯酰胺凝胶园盘电泳(SDS—PAGE)是根据尿中各种蛋白质分子量大小,在聚丙烯酰胺凝胶中泳动速度不同,而达到区分各种不同分子量蛋白质。正常尿蛋白按分子量高低分免疫球蛋白、转铁蛋白、白蛋白及溶菌酶,共4条蛋白带。若与对照管相比,蛋白带数增多或增宽均为阳性。由于白蛋白属中分子,所以在凝胶条中白蛋白带以上部分增加的蛋白带,称高分子尿蛋白,白蛋白以下部分增加的蛋白带称低分子尿蛋白。若在一个凝胶条中,高、中、低三种分子蛋白带均同时增加者,称混合型尿蛋白。我们按照上海第三人民医院检查方法,略加改进,对430例肾脏病进行尿 SDS—PAGE 检查,其中肾炎组348例,阳性283例,阳性率为81.3%,包括急性肾炎5例,阳性1例(20%),为低分子尿
