维甲酸类和维生素D_3能经由特异性核受体调节多种细胞(包括造血细胞和癌细胞)的生长和分化。体外研究提示维生素A、D衍生物能在细胞水平发挥协同作用,从而扩增激素信号。作者联用阿维A(acitretin,依曲替酸)和维生素D衍生物骨化三醇(1,25-二羟胆骨化醇)治疗1例皮肤T细胞淋巴瘤病人。 患者女,79岁,因腹部、躯干、臂、腿部多发性红斑浸润性结节(直径0.5cm~2cm)而就诊。皮肤活检显示部分肉芽肿性多形性皮肤浸润,主要由小的辅助性T淋巴细胞组成,集中于皮肤附属器周围,有少量不典型细胞,无明显嗜表
Retinoic acid and vitamin D 3 can regulate the growth and differentiation of various cells, including hematopoietic cells and cancer cells, via specific nuclear receptors. In vitro studies suggest that vitamin A, D derivatives can exert a synergistic effect at the cellular level, thereby amplifying hormone signals. The authors treated a patient with cutaneous T cell lymphoma with acitretin and vitamin D derivative calcitriol (1,25-dihydroxycholecalciferol). A 79-year-old female patient presented with multiple erythematous infiltrative nodules (0.5 cm to 2 cm in diameter) on the abdomen, torso, arms, and legs. Skin biopsy revealed some of the granulomatous pleomorphic skin infiltrate, consisting mainly of small helper T-lymphocytes, concentrated around the skin appendages, with a small number of atypical cells, with no apparent appearances.
