目的通过检测分析手足口病患儿N末端脑钠肽前体(NT-pro BNP)浓度水平来判断患儿的病情轻重程度及预后,以指导临床治疗。方法选择2013年10月至2015年4月在郑州市儿童医院住院确诊的手足口病患儿轻度组60例、中度组72例、重度组56例,诊断标准以卫生部制定的《手足口病诊疗指南(2010年版)》为依据,对照组正常体检儿童50例。采用电化学发光法检测NT-pro BNP,以125pg/ml作为决策阈值。NT-pro BNP<125pg/ml排除心衰;若NT-pro BNP>125pg/ml提示有心脏功能障碍。结果 NT-pro BNP浓度在手足口病患儿轻度组与对照组差异无统计学意义,中度组与重症患者组显著高于轻度与对照组(P<0.01)。结论血清NT-pro BNP浓度可为手足口病患儿病情评估提供参考依据,将其作为早期识别重症患者的重要依据,具有广泛的临床应用价值。
Objective To detect the severity and prognosis of children with hand-foot-mouth disease by detecting the concentration of N-terminal pro-brain natriuretic peptide (NT-pro BNP) in order to guide the clinical treatment. Methods From October 2013 to April 2015, 60 children with HFMD admitted to Children’s Hospital of Zhengzhou City were selected as mild group (72 cases) and moderate group (56 cases). The diagnostic criteria were “Hand-Foot Stomatology diagnosis and treatment guidelines (2010 version) ”as the basis, the control group of normal physical examination children 50 cases. NT-pro BNP was detected by electrochemiluminescence with 125 pg / ml as a decision threshold. NT-pro BNP <125 pg / ml to exclude heart failure; NT-pro BNP> 125 pg / ml suggests heart dysfunction. Results The concentration of NT-pro BNP was not significantly different between mild group and control group. The moderate group and severe group were significantly higher than those of mild group and control group (P <0.01). Conclusion Serum NT-pro BNP concentration may provide a reference for the assessment of the disease in children with HFMD. It can be used as an important basis for early identification of critically ill patients and has a wide range of clinical value.
